摘要:
目的探讨经颅磁刺激（transcranial magnetic stimulatron，TMS）联合康复训练对慢性精神分裂症患者阴性症状的影响。 方法将86例慢性精神分裂症患者随机分为联合组、TMS组各43例，两组均给予TMS治疗，联合组在TMS的基础上加用康复训练治疗。分别于治疗前、治疗2周后，使用阳性与阴性症状量表（positive and negative syndrome scale，PANSS）、阴性症状量表（negative syndrome scale，SANS）、住院精神病患者康复疗效评定量表（inpatient psychiatric rehabilitation efficacy rating scale，IPROS）评估两组患者症状变化，并对比其临床疗效及不良反应发生情况。 结果治疗2周后两组患者PANSS量表阴性因子分及总分均较治疗前下降，联合组低于TMS组（P＜0.05）。治疗2周后两组患者SANS量表各分量表评分及总分均较治疗前下降，联合组低于TMS组（P＜0.05）。治疗2周后两组患者IPROS量表各分量表评分及总分均较治疗前下降，联合组IPROS量表中生活能力、社交能力、关心和兴趣分量表评分及总分均低于TMS组（P＜0.05）。总有效率联合组为51.16%，高于TMS组的30.23%（P＜0.05）。联合组治疗期间不良反应发生率为9.30%，低于TMS组的13.95%，但差异无统计学意义（P＞0.05）。 结论在TMS的基础上联合康复训练能够改善慢性精神分裂症患者阴性症状，提高早期治疗效果，且安全性良好。
Abstract:
ObjectiveTo survey the effects of transcranial magnetic stimulation (TMS) combined with rehabilitation training on negative symptoms in patients with chronic schizophrenia. MethodsAll 86 patients were randomized into the combination group and TMS group, 43 cases in each group, both groups accepted TMS therapy, and the combination group received rehabilitation therapy based on TMS. PANSS and SANS, IPROS were applied to assess the changes of symptoms of the patients in both groups before the treatment and after two weeks of treatment respectively, to compare clinical effects and the incidence of adverse reaction. ResultsThe scores of negative factors among PANSS scale and total scores in both groups after treatment were lower than these before the treatment, the combination group was lower than TMS group (P<0.05). The scores of each subscale of SANS scales and total scores of both groups declined after two weeks of treatment than these before treatment, the combination group was lower than TMS group (P<0.05). After two weeks of treatment, the scores of each subscale of IPROS scales and total scores of both groups reduced compared with these before treatment, the combination group was lower than TMS group in the scores of living ability, social ability, care and interest in each subscale of IPROS scales and total scores (P<0.05). Total effective rate of the combination group was 51.16%, higher than 30.23% of TMS group (P<0.05). The incidence of adverse reaction of the combination group was 9.30% during therapeutic period, lower than 13.95% of TMS group (P>0.05). ConclusionBased on TMS, rehabilitation training could further improve negative symptoms in patients with chronic schizophrenia, raise therapeutic effects early with good safety.
