摘要:
目的：探讨心电图诊断高血钾症的临床价值。方法：对70例高血钾症患者的心电图表现与血清钾浓度的变化进行对比分析。结果：心电图异常改变和血清钾浓度测定存在一定的差异，对肾功能不全所致高血钾症诊断有较好的一致性。当血清钾浓度超过5.5 mmol/L时，心电图表现为帐蓬状高耸T波，基底变窄，Q-T间期缩短，随着血清钾浓度的继续增高，心电图异常发生率亦相应增加，主要是细胞外钾离子浓度增高引起的心电图改变。结论：心电图是评价高血钾症病情程度的重要手段，为临床诊断及治疗提供参考。
Abstract:
Objective: To explore clinical value of electrocardiogram (ECG) in diagnosing hyperkalemia. Methods: The changes of potassium concentrations in the serum and ECG changes of 70 patients with hyperkalemia were compared and analyzed. Results: The manifestation of abnormal ECG is not totally related to serum potassium density, however, it shows a good consistence to hyperkalemia caused by renal failure. When serum potassium density is higher than 5.5mmol/L, ECG reveals a tall T wave with a broad base, a shorten Q-T interval, as serum potassium density is increased continually, the incidence of abnormal ECG is raised, which is mainly caused by the density increase of extracellular potassium-ion. Conclusion: ECG is an important method for diagnosing the severity of hyperkalemia, it could provide reference for clinical diagnosis and treatment.
