摘要:
目的：探讨骨伤科骨折并发脾破裂的责任制护理措施。方法：回顾分析2009年3月至2015年6月收住的46例骨折并发脾破裂患者的责任制护理临床资料，其中真性脾破裂44例，中央型脾破裂1例，被膜下脾破裂1例。结果：经实施责任制护理，46例患者均能在第一时间做出准确诊断及治疗，42例转普外科，及时手术，治愈出院，无严重并发症发生；2例（中央型脾破裂及被膜下脾破裂各1例）行保守治疗，治愈出院；2例因确诊时间较晚,休克导致多器官功能衰竭，抢救无效死亡。结论：对骨折并发脾破裂患者应用责任制护理，有利于早期诊断、精心护理、紧急救治，有利于减少漏诊误诊及并发症，提高救治成功率。
Abstract:
Objective: To explore the measures of primary nursing for the patients suffering from bone fracture complicated with splenic rupture. Methods: Clinical data of 46 patients admitted to hospital from March, 2009 to June, 2015 in orthopedic department were retrospectively analyzed, there were 44 patients with true splenic rupture, one case with cental splenic rupture and one case with subcapsular splenic repture. Results: After primary nursing was implemented, all 46 patients accepted accurate diagnosis and treatment timely, 42 patients transferred to surgery department, received the surgery, cured and discharged, and no severe complications happened; two cases (one case with cental splenic rupture and one case with subcapsular splenic repture) were observed and they accepted conservative treatment, cured and discharged; two patients were suffering from shock and multiple organ failure since they came to hospital too late, all rescue measures failed and the patients died. Conclusion: Primary nursing applied to the patients suffering from bone fracture complicated with splenic repture is helpful to early diagnosis, careful nursing, emergency aid and treatment, reduce missed diagnosis and misdiagnosis, the complications, improve success rate and save the patients.
