摘要:
目的：观察抗痫煎剂治疗症状性癫痫全身性强直-阵挛发作的临床疗效、中医辨证分型的疗效差异以及脑电图的改变情况。方法：将症状性癫痫全身性强直-阵挛发作患者124例分为2组各62例，对照组患者口服卡马西平片0.1～0.2 g/次，3次/d；治疗组在对照组治疗的基础上加用抗痫煎剂，早晚各服100 mL，2组均治疗3个月为1个疗程，1个疗程后进行疗效评定。结果：2组总有效率比较差异有统计学意义(P＜0.01)，治疗组优于对照组；同一证型治疗组与对照组有效率比较，风痰闭阻型差异有统计学意义(P＜0.01)，痰火扰神型差异有统计学意义(P＜0.05)，血虚生风型、心脾两虚型、肝肾阴虚型差异均无统计学意义(P＞0.05)；在改善脑电图方面治疗组亦优于对照组(P＜0.05)。结论：抗痫煎剂对症状性癫痫的全身性强直-阵挛发作型有较好的抗痫作用，以风痰闭阻型、痰火扰神型尤佳。
Abstract:
Objective: To observe clinical effects of KangXian decoction in treating symptomatic epilepsy of generalized tonic-clonic pattern, the difference between curative effects of syndrome differentiation and patterns and the changes of EEG. Methods: All 124 patients were randomized into two groups, 62 cases each group, the control group took carbamazepine tablets orally, 0.1~0.2 g each time, three times per day; the treatment group accepted KangXian decoction on the basis of the therapy the control group received, 100mL, in the morning and evening, three months were one course of the treatment for both groups, clinical effects were evaluated in one course of the treatment. Results: The treatment group was superior to the control group in the comparison of total effective rates between both groups, and the difference was significant (P＜0.01); the difference showed statistical meaning in the comparisons of the effective rates between the treatment group and the control group in the same pattern: wind-phlegm obstruction pattern (P＜0.01), the difference had significant meaning in the comparison of the pattern of phlegm-fire disturbing mind (P＜0.05), the difference had no significant meaning in the comparisons of blood deficiency causing wind, dual deficiency of heart-spleen, liver-kidney Yin-deficiency (P＞0.05); the treatment group was superior to the control group in improving EEG (P＜0.05). Conclusion: KangXian decoction is effective in the treatment of symptomatic epilepsy of generalized tonic-clonic pattern, especially wind-phlegm obstruction and phlegm-fire disturbing mind pattern.
