摘要:
目的：观察桃红四物汤对老年股骨转子间骨折股骨近端防旋髓内钉内固定术后隐性失血的影响。方法：将90例患者按就诊的先后顺序随机分为观察组、对照组各45例。所有患者均在椎管内麻醉下行股骨近端防旋髓内钉内固定术，术毕给予抗炎、止痛、消肿、抗凝、促进骨愈合、预防并发症等对症治疗。观察组于术后当天服用桃红四物汤，每日1剂，水煎早晚分服，连续服用7天。比较2组治疗后血红蛋白及血小板计数，引流管留置时间及总引流量，术后并发症情况。结果：术后第8天血红蛋白水平及血小板计数观察组均高于对照组，差异有统计学意义(P＜0.05)。术后引流管留置时间观察组短于对照组，差异有统计学意义(P＜0.05)；总引流量观察组少于对照组，差异有统计学意义(P＜0.05)。术后并发症发生率观察组为2.2%（1/45），对照组为24.4%（11/45），2组比较差异有统计学意义(P＜0.05)。结论：使用桃红四物汤治疗，能有效减少老年股骨转子间骨折股骨近端防旋髓内钉内固定术后隐性失血，维持血液循环功能稳定，减少并发症，促进患者术后恢复。
Abstract:
Objective: To discuss the effects of TaoHong SiWu decoction on postoperative hidden hemorrhage of senile femoral intertrochanteric fracture who underwent internal fixation of proximal femoral nail anti-rotation (PFNA). Methods: Ninety patients were randomized into the observation group and the control group according to the visiting order, 45 cases each group. All the patients accepted internal fixation of PFNA under intravertebral anesthesia, and symptomatic treatment of anti-inflammation, stopping pain, reducing the swelling, anticoagulation, promoting bone healing, preventing the complications after the operation. The observation group began to take TaoHong SiWu Tang at the day after the operation simultaneously, one dose each day, water decocted and taking in the morning and evening, for seven days consecutively. Results: The observation group was higher than the control group in the level of hemoglobin and blood platelet counting, and the difference had statistical meaning (P＜0.05). The observation group was shorter than the control group in the retention time of drainage tube after the operation, and the difference showed statistical meaning (P＜0.05); the observation group was less than the control group in total drainage, and the difference was statistically significant (P＜0.05). Postoperative complications incidences of the observation group was 2.2% (1/45), lower than 24.4% (11/45) of the control group, and the difference had statistical meaning (P＜0.05). Conclusion: Treating postoperative hidden hemorrhage of senile femoral intertrochanteric fracture who underwent internal fixation of PFNA by TaoHong SiWu decoction could effectively reduce hidden hemorrhage, maintain the stability of blood circulation, reduce the complications and promote postoperative rehabilitation of the patients.
