摘要:
目的：观察益肾祛痹汤联合西药治疗类风湿性关节炎(RA)的临床疗效。方法：将120例患者随机分为观察组、对照组各60例，对照组采用甲氨蝶呤、柳氮磺胺吡啶和尼美舒利治疗，观察组在对照组治疗的基础上服用益肾祛痹汤，每日1次，水煎分服。2组均连续治疗2个月。观察2组临床疗效，主要相关体征。类风湿相关指标及副反应情况。结果：总有效率观察组为93.3%，对照组为76.7%，2组比较差异有统计学意义(P＜0.05)。晨僵时间、关节压痛指数、关节肿胀指数、双手平均握力等指标治疗前后2组组内比较，差异均有统计学意义(P＜0.05)；治疗后组间比较，差异也有统计学意义(P＜0.05)。C反应蛋白(CRP)、类风湿因子定量(RF)、血沉(ESR)、血管内生长因子(VEGF)、血浆纤维蛋白原(FG)治疗前后2组组内比较，差异均有统计学意义(P＜0.05)；治疗后组间比较，差异也有统计学意义(P＜0.05)。观察组在治疗过程中出现轻度副反应2例，治疗过程未发现肝肾功能损害，血尿常规均未发生明显变化。结论：益肾祛痹汤联合西药治疗RA安全、疗效显著。
Abstract:
Objective: To observe curative effects of kidney-tonifying arthralgia-eliminating decoction combined with western medicine in treating rheumatoid arthritis(RA). Methods: All 120 patients were randomized into the observation group and the control group, 60 cases each group, the control group were treated by metho-trexate, salazosulfapyridine (SASP) and nimesulide, and the observation group took kidney-tonifying arthralgia-eliminating decoction based on the therapy that the control group accepted, one dose each day, water decoction. Both groups were treated for two months consecutively. Clinical effects, main related body signs, the related indexes of RA and the conditions of adverse reaction of both groups were observed. Results: Total effective rate of the observation group was 93.3%, higher than 76.7% of the control group, and the difference had statistical meaning (P＜0.05). The difference was statistically significant in the comparisons of morning stiffness time, joint tenderness index, joint swelling index, average grip strength of both hands and others within two groups between before and after the treating (P＜0.05); there was a significant difference in the comparisons between both groups after treating (P＜0.05). When CRP, RF, ESR, VEGF and FG were compared within two groups before and after treating, the difference had statistical meaning (P＜0.05); when the indexes were compared between both groups after treating, the difference showed statistical meaning (P＜0.05). There were two cases with mild adverse reaction during the therapeutic course in the observation group, no liver and kidney functional damage was found during the entire course, no obvious changes has been seen in the blood and urinary routine tests. Conclusion: Kidney-tonifying arthralgia-eliminating decoction combined with western medicine is safe and effective in treating RA.
