摘要:
目的：对比小针刀和常规针刺或腰椎牵引疗法治疗腰椎间盘突出症(LDH)的疗效和安全性，探讨小针刀治疗LDH的可行性。方法：将89例患者随机分为观察组45例和对照组44例。观察组采用小针刀治疗，5天治疗1次；对照组采用常规针刺或腰椎牵引疗法治疗，隔天治疗1次。2组均治疗20天，比较2组临床疗效、视觉疼痛模拟(VAS)评分、白细胞介素1β(IL-1β)和肿瘤坏死因子α(TNF-α)水平及安全性。结果：总有效率观察组为95.5%，对照组为78.6%，2组比较差异有统计学意义(P＜0.05)。疼痛VAS评分治疗前后2组组内比较，差异有统计学意义(P＜0.05)；治疗后2组组间比较差异有统计学意义(P＜0.05)。IL-1β和TNF-α水平治疗前后2组组内比较，差异有统计学意义(P＜0.05)；治疗后2组组间比较，差异有统计学意义(P＜0.05)。2组患者均无严重不良反应，脱落率比较，2组差异无统计学意义(P＞0.05)。结论：小针刀治疗LDH疗效良好，安全可行，其部分机制可能是通过抑制炎症因子IL-1β和TNF-α的释放而实现的。
Abstract:
Objective：To study the feasibility of small needle-knife in treating LDH by comparing clinical effects and the safety of small needle-knife and conventional needling or lumbar traction therapy in the treatment for LDH. Methods: All 89 patients were divided into 45 cases of the observation group and 44 cases of the control group. The observation group were treated by small needle-knife, once each five days; the control group adopted conventional needling or lumbar traction therapy, once every other day, twenty days were one course, the safety, the levels of TNF-α, IL-1β, VAS and clinical effects of both groups were compared. Results: Total effective rate of the observation group was 95.5%, superior to 78.6% of the control group(P＜0.05); The difference was statistically significant in the conparison of VAS scores within two groups before and after treating(P＜0.05); the difference had statistical meaning in the comparison between both troups after treating (P＜0.05); the levels of TNF-α and IL-1β decreased compared with before treating(P＜0.05); and the observation group was superior to the control group, and the difference had statistical meaning(P＜0.05). No severe adverse reaction happened, and the difference had no statistical meaning in the comparisons of expulsion rate (P＞0.05). Conclusion: Small needle-knife is effective, safe and feasible in treating LDH, the part of the mechanism might be realized by inhibiting the release of TNF-α and IL-1β.
