摘要:
目的：观察平陈汤对脾虚痰湿型2型糖尿病前期患者的临床疗效。方法：将100例脾虚痰湿型2型糖尿病前期患者随机分为观察组和对照组各50例。2组均给予糖尿病教育+饮食+运动的基础干预，观察组同时服用平陈汤配方颗粒，开水冲服每次200 mL，2次/d，2组均治疗8周。观察2组临床疗效及治疗前后中医证候积分、空腹血糖、胰岛素抵抗指数(IR)、体质指数(BMI)、腰围、总胆固醇(TCHO)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、血压和血尿酸水平的变化情况。结果：总有效率观察组为84.75%，对照组为55.32%，2组比较差异有统计学意义(P＜0.05)。中医证候总积分、空腹血糖、IR、BMI、腰围、TC、TG、HDL-C、LDL-C治疗前后组内比较，差异有统计学意义(P＜0.05)；治疗后组间比较差异也有统计学意义(P＜0.05)。收缩压、舒张压、血尿酸治疗前后本组组内比较，差异有统计学意义(P＜0.05)；治疗后组间比较差异无统计学意义(P＞0.05)。结论：平陈汤治疗脾虚痰湿型2型糖尿病前期患者，可以较好改善患者的临床症状及胰岛素抵抗指数等相关指标，延缓糖调节受损(IGR)向2型糖尿病的进一步发展。
Abstract:
Objective: To survey curative effects of PingChen decoction in the treatment for the patients with type two prediabetes of spleen deficiency phlegm damp pattern. Methods: All 100 patients were randomized into the observation group and the control group, 50 cases each group. Both groups accepted basic intervention including diabetic education+diet+exercise, and the observation group took the granules of PingChen decoction, 200 mL each time, twice each day, both groups were treated for eight weeks. The levels of blood uric acid (BUA), blood pressure, low density lipoprotein-cholesterol (LDL-C), high density lipoprotein-cholesterol (HDL-C), triglyceride (TG), total cholesterol (TCHO), waistline, body mass index (BMI), insulin resistance (IR), fasting blood-glucose (FBG) and TCM syndrome integrals and clinical effects of both groups were observed before and after treating. Results: Total effective rate of the observation group was 84.75%, higher than 55.32% of the control group, and the difference showed statistical meaning (P＜0.05). The difference was statistically significant in the comparisons of TCM syndrome integrals, FBG, IR, BMI, waistline, TC, TG, HDL-C and LDL-C between before and after treating within the group (P＜0.05); the difference showed statistical meaning in the comparisons between both groups after treating (P＜0.05). There was a significant difference in the comparisons of systolic pressure, diastolic pressure and BUA between before and after treating within the group (P＜0.05); the difference had no statistical meaning in the comparisons between both groups after treating (P＞0.05). Conclusion: PingChen decoction is effective in clinical intervention for the patients with type two prediabetes of spleen deficiency phlegm damp pattern, it could improve clinical symptoms and the related indexes of IR, and delay further progress of impaired glucose regulation (IGR) into type two diabetes mellitus.
