摘要:
目的：观察针灸结合藤黄健骨胶囊治疗神经根型颈椎病的疗效及对血清白细胞介素6(interleukin-6，IL-6)、肿瘤坏死因子α(tumor necrosis factor α，TNF-α)和血液流变学的影响。方法：将神经根型颈椎病患者112例按照随机数字表法分为研究组和对照组各56例，两组均给予针灸治疗，对照组在此基础口服塞来昔布胶囊每次200 mg，每日1次，同时口服甲钴胺胶囊每次0.5 mg，每日3次，持续治疗2周。研究组在此基础上口服藤黄健骨胶囊每次4～6粒，每日2次，两组均持续治疗2周。观察两组患者治疗前后中医证候积分、颈椎病症状量表评分、血清IL-6、TNF-α水平及血液流变学指标，评价两组临床疗效。结果：治疗后两组患者中医证候积分均降低(P＜0.05)，且研究组低于对照组(P＜0.05)；治疗后两组患者症状、生活和工作能力、体征、手功能等评分均升高(P＜0.05)，且研究组高于对照组(P＜0.05)；治疗后两组患者血清IL-6、TNF-α水平及血浆黏度、全血高切黏度、全血低切黏度、纤维蛋白原、红细胞压积均降低(P＜0.05)，且研究组低于对照组(P＜0.05)；研究组总有效率高于对照组(P＜0.05)。结论：针灸结合藤黄健骨胶囊可有效降低神经根型颈椎病患者血清IL-6、TNF-α水平，改善患者临床症状及血液流变学指标。
Abstract:
Objective: To observe clinical effects of acupuncture and Tenghuang Jiangu capsules in the treatment for radicular type cervical spondylosis, and the effects on serum interleukin-6 (IL-6), tumor necrosis factor alpha(TNF-α) and blood rheology. Methods: All 112 patients were allocated to the study group and the control group, 56 cases each group. The two groups were given acupuncture, the control group took celecoxib capsules, 200mg each time, once each day, simultaneously, mecobalamine capsules orally, 0.5mg each time, three times each day, for two weeks continuously. The study group took Tenghuang Jiangu capsules orally, four to six pills each time, twice each day, both groups were treated for two weeks continously. To observe TCM syndrome scores, cervical spondylosis symptom scale score, the levels of IL-6 and TNF-α, and blood rheology indexes before and after treating in both groups, and assess clinical effects of both groups. Results: After treating, TCM syndrome scores of two groups decreased(P＜0.05), and the study group was lower than the control group(P＜0.05). After treating, the scores of symptoms, life ability, work ability, physical signs, hand function and others rose in both groups (P＜0.05), and the study group was higher than the control group(P＜0.05); after treating, serum IL-6 and TNF-α levels, plasma viscosity, whole blood high shear viscosity, whole blood low shear viscosity, contractinogen and hematocrit in both groups lowered (P＜0.05), and the study group was lower than the control group (P＜0.05); total effective rate of the study group was higher than that of the control group(P＜0.05). Conclusion: Acupuncture and Tenghuang Jiangu capsules in the treatment for radicular type cervical spondylosis could effectively reduce the levels of IL-6 and TNF-α, improve clinical symptoms and the indexes of blood rheology.
