摘要:
目的观察宣白承气汤联合西药治疗呼吸机相关性肺炎（ventilator-associated pneumonia，VAP）的临床疗效，探讨宣白承气汤治疗VAP的临床效果及作用机制。 方法选择VAP患者82例，随机分为观察组和对照组，每组41例。对照组采用西医常规治疗，观察组在此基础上加用宣白承气汤鼻饲。治疗7天后观察两组临床疗效，炎症指标水平［外周血白细胞（white blood cell，WBC）、C-反应蛋白（C-reactive protein，CRP）、降钙素原（procalcitonin，PCT）水平］氧合指数，肺部感染评分（clinical pulmonary infection score，CPIS）及中医证候积分。 结果总有效率观察组为90.24%（37/41），对照组为78.05%（28/41），两组比较差异有统计学意义（P＜0.05）。观察组外周血WBC、CRP、PCT水平、氧合指数水平，CPIS评分及中医证候积分较对照组有明显下降，两组比较差异有统计学意义（P＜0.05）。 结论宣白承气汤联合西药能显著改善患者临床症状，提高氧合指数，降低外周血WBC、CRP、PCT水平，还能降低肺部感染评分和中医证候积分，提示通过宣白承气汤能控制患者肺部感染，减轻炎性反应，从而改善患者的呼吸功能。
Abstract:
ObjectiveTo survey clinical effects of Xuanbai Chengqi Tang and Western medicine in the treatment of VAP, discuss its clinical effects and mechanism. MethodsAll 82 VAP patients were chosen, randomized into the observation group and the control group, 41 cases each group. The control group adopted conventional therapy of Western medicine, and the observation group accepted nasogastric feeding of Xuanbai Chengqi Tang based on Western medicine. After treating for seven days, to observe clinical effects, the levels of WBC, CRP and PCT, the levels of oxygenation indexes, CPIS scores and TCM syndrome integrals. ResultsTotal effective rate of the observation group was 90.24% (37/41), higher than 78.05% (28/41) of the control group, the difference had statistical meaning between both groups (P<0.05). The levels of WBC, CRP and PCT, the levels of oxygenation indexes, CPIS scores and TCM syndrome integrals of the observation group reduced notably than these of the control group, and the difference had statistical meaning (P<0.05). ConclusionXuanbai Chengqi Tang and Western medicine could improve clinical symptoms notably, raise oxygenation indexes, reduce the levels of WBC, CRP and PCT in peripheral blood, reduce pulmonary infection score and TCM syndrome integrals, which suggests that Xuanbai Chengqi Tang could improve respiratory function through controlling pulmonary infection and relieving inflammatory reaction, therefore to improve respiratory function.
