摘要:
目的观察强骨胶囊联合早期量化功能锻炼治疗老年膝关节骨性关节炎（knee osteoarthritis，KOA）的疗效，及其对核因子κB（nuclear factor-κB，NF-κB）信号通路的影响。 方法选择KOA患者102例，按照随机数字法分为观察组和对照组，每组51例。对照组予以常规综合治疗，观察组在对照组的基础上予以强骨胶囊联合早期量化功能锻炼治疗。比较两组临床疗效，观察两组治疗前后膝关节功能、证候积分、活动及休息时骨内压、白细胞介素6（interleukin-6，IL-6）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、高敏C反应蛋白（hypersensitivity C-reactive protein，hs-CRP）、核因子抑制蛋白κB（inhibitor kappa B-α，IκB-α）、P65和P50水平的变化。 结果观察组总有效率［96.08%（49/51）］明显高于对照组［80.39%（41/51）］（P＜0.05）。治疗后两组疼痛、僵硬、日常活动、中医证候积分、活动时骨内压、休息时骨内压、IL-6、TNF-α、hs-CRP、IκB-α、P65和P50水平较治疗前明显降低（P＜0.05），而屈曲度较治疗前明显升高（P＜0.01），且观察组改善较对照组更明显（P＜0.05）。 结论强骨胶囊联合早期量化功能锻炼治疗老年KOA疗效显著，可有效改善患者症状，促进膝关节功能恢复，其机理可能与抑制NF-κB通路有关。
Abstract:
ObjectiveTo observe clinical effects of bone-strengthening capsules and early quantitative functional exercise in treating the elderly with knee osteoarthritis (KOA), and its influence on NF-κB signaling pathway. MethodsAll 102 patients were chosen, allocated to the observation group and the control group according to random number table method, 51 cases each group. The control group accepted conventional comprehensive therapy, and the observation group bone-strengthening capsules and early quantitative functional exercise on the foundation of conventional comprehensive therapy. To compare total effective rate of both groups after treating, to observe knee function, TCM syndrome integrals, intraosseous pressure during exercise and at rest, the levels of IL-6, TNF-α, hs-CRP, IκB-α, P65 and P50 between both groups before and after treating. ResultsTotal effective rate of the observation group was 96.08% (49/51), notably superior to 80.39% (41/51) of the control group (P<0.05). After treating, the scores of pain, stiffness, daily activities, TCM syndrome integrals, intraosseous pressure during exercise and at rest, the levels of IL-6, TNF-α, hs-CRP, IκB-α, P65 and P50 lowered notably compared with these before treating (P<0.05), while flexion degree increased than that before treating (P<0.01), the improvements of the observation group were more notable than these of the control group (P<0.05). ConclusionBone-streng-thening capsules and early quantitative functional exercise are effective in treating the elderly with KOA, it could improve the symptoms of the patients, promote the functional recovery of knee joint, and its mechanism might be related to inhibiting NF-κB pathway.
