摘要:
目的探讨黄芪注射液对病毒性心肌炎患者外周血辅助性T细胞17（T helper cell 17，Th17）及调节性T细胞（regulatory cell，Treg）细胞亚群的影响。 方法将140例病毒性心肌炎患者根据随机数字表法分为观察组和对照组各70例，对照组予常规治疗，观察组在对照组基础上静脉滴注黄芪注射液，疗程为2周。观察治疗前后两组患者外周血Th17及Treg细胞亚群、相关炎性细胞因子［白细胞介素6(interleukin-6，IL-6)、白细胞介素17(interleukin-17，IL-17)及转化生长因子β（transforming growth factor-β,TGF-β ）］水平与心肌损伤标志物［（肌酸激酶同工酶（creatine kinase isoenzyme，CKMB）、心肌肌钙蛋白I（cardiac troponin I，cTnI）、肌红蛋白（myoglobin，MYO）］水平；观察两组患者临床疗效及不良反应发生情况。 结果与治疗前比较，两组患者治疗后外周血Th17细胞、Th17/Treg比值、IL-6及IL-17水平降低（P＜0.05），观察组降低更明显；Treg细胞亚群和TGF-β水平升高，观察组升高更明显（P＜0.05）。治疗后观察组患者血清CKMB、cTnI及MYO水平低于对照组（P＜0.05）；观察组总有效率（92.9%）高于对照组（82.9%）（P＜0.05）；不良反应两组比较差异无统计学意义（P＞0.05）。 结论黄芪注射液可改善病毒性心肌炎患者外周血Th17与Treg细胞亚群的失衡，抑制下游炎症反应激活，减轻心肌损伤。
Abstract:
ObjectiveTo discuss the effects of Huangqi injection on Th17 and Treg cell subsets in peripheral blood of patients with viral myocarditis. MethodsAll 140 patients were divided into the observation group and the control group according to random number table method, 70 cases in each group, the control group adopted conventional therapy, and the observation group accepted intravenous injection of Huangqi injection based on routine therapy, the course of the disease was two weeks. To observe the levels of Th17 and Treg cell subsets in peripheral blood before and after the treatment, the relevant inflammatory cytokines [IL-6, IL-17 and TGF-β], the levels of myocardial injury marker including CKMB, cTnI and MYO; to observe clinical effects and the incidence of adverse reaction between both groups. ResultsCompared with before the treatment, the levels of Th17 cells, Th17/Treg ratio, the levels of IL-6, IL-17 were reduced in both groups after the treatment (P<0.05), the decrease of the observation group was more notable; the levels of Treg cell subsets and TGF-β were increased, and the increase of the observation group was more noticeable (P<0.05). After the treatment, the levels of CKMB, cTnI and MYO of the observation group were lower than these of the control group (P<0.05); total effective rate of the observation group was 92.9%, higher than 82.9% of the control group (P<0.05); the difference had no statistical meaning in adverse reaction between both groups (P>0.05). ConclusionHuangqi injection could balance Th17 and Treg cell subsets in peripheral blood, inhibit downstream inflammatory activation and relieve myocardial injury.
