摘要:
目的探讨补肾健脾壮骨方联合钙尔奇D治疗脾肾两虚型原发性骨质疏松症的临床疗效及对血清钙调素（calmodulin，CaM）、转化生长因子β1（transforming growth factor-β1,TGF-β1)水平的影响。 方法将脾肾两虚型原发性骨质疏松症患者108例按随机数字表法分为对照组和试验组各54例，对照组予六味地黄丸联合钙尔奇D片，试验组予补肾健脾壮骨方联合钙尔奇D片，两组均治疗6个月后观察患者临床疗效、中医证候积分、腰椎L2～4及双侧股骨颈骨密度、血液流变学指标与血清CaM、TGF-β1水平。 结果试验组总有效率为94.12%，高于对照组的80.77%（P＜0.05）；治疗后中医证候积分试验组总评分低于对照组（P＜0.05），其中腰脊酸痛、腰膝酸软、肢体倦怠评分低于对照组（P＜0.05），而头晕目眩评分两组相比差异无统计学意义（P＞0.05）；治疗后试验组患者腰椎、双侧股骨颈骨密度高于对照组（P＜0.05），血浆黏度、红细胞聚集指数及血清TGF-β1水平低于对照组（P＜0.05），CaM水平高于对照组（P＜0.05）。 结论补肾健脾壮骨方联合钙尔奇D能够防止脾肾两虚型原发性骨质疏松症患者骨密度下降，改善血液流变性及骨代谢指标，效果优于六味地黄丸联合钙尔奇D。
Abstract:
ObjectiveTo discuss clinical effects of tonifying-kidney-spleen strengthening-bone prescription (TKSSBP) and caltrate D in the treatment of primary osteoporosis of spleen-kidney dual deficiency pattern and its influence on the levels of CaM and TGF-β1. MethodsAll 108 patients were allocated to the control group and the experiment group by using random number table method, with 54 cases in each group, the control group was given Liuwei Dihuang pills and caltrate D, and the experiment group TKSSBP and caltrate D, both groups were treated for six months, to observe clinical effects, TCM syndrome integrals, bone mineral density (BMD) of lumbar vertebra L2~L4 and bilateral femoral necks, hemorheology indexes and the levels of serum CaM and TGF-β1 between both groups. ResultsTotal effective rate of the experiment group was 94.12%, higher than 80.77% of the control group (P<0.05); total scores of TCM syndrome integrals of the experiment group were lower than these of the control group (P<0.05), among which, the scores of lumbar and spinal ache, soreness and weakness of waist and knees were lower than these of the control group (P<0.05), while the difference had no statistical meaning in the scores of light-headedness between both groups (P>0.05); the experiment group was higher than the control group in BMD of lumbar vertebra and bilateral femoral necks (P<0.05); the experiment group was lower than the control group in plasma viscosity, erythrocyte aggregation index and the levels of TGF-β1 (P<0.05), higher than the control group in the levels of CaM (P<0.05). ConclusionTKSSBP and caltrate D in the treatment of primary osteoporosis of spleen-kidney dual deficiency pattern could prevent the decrease of BMD, improve hemorheology indexes and bone metabolism indexes, and its effects are superior to these of Liuwei Dihuang pills and caltrate D.
