摘要:
目的对甘肃省某三甲公立中医医院各临床科室的运营效率进行评价，为医院和科室管理提供决策参考。 方法调取该院2016—2021年医疗及人力资源数据，运用数据包络分析（Data envelopment analysis，DEA）中的BBC模型进行卫生资源投入产出效率评价，选取Mann-Whitney检验，探索影响DEA有效性的关键因素。 结果截止2021年，全院临床科室共有医务人员1091人，共有床位2084张，门诊接诊患者1 094 387人次，住院患者81 417人次，平均住院日（12.35±2.85）天。2016—2021年该院临床科室综合技术效率平均值为0.810，纯技术效率为0.908，规模效率为0.886，内科组、儿科组和其他组为DEA有效，其余各组均为DEA无效；比较DEA有效组和无效组各项卫生资源投入产出之间的差异，两组之间除床位数没有差异（P＞0.05）外，其余各项均存在统计学差异（P＜0.05）。 结论医院总体纯技术效率较高，但由于资源利用水平不高，造成医院综合技术效率不足，DEA有效的临床科室应通过科学计算，适度扩大投入产出，DEA无效的临床科室应合理调整医疗卫生资源配置，引进学科特色技术，增加卫生资源的产出，形成最优生产模式达到医院和科室的共同高效发展。
Abstract:
ObjectiveTo provide the decision-making reference for hospital and department management by evaluating the operation efficiency of clinical departments in a public tertiary TCM hospital in Gansu province. MethodsMedical and human resources data from 2016 to 2021 were retrieved, and the BBC model in data envelopment analysis (DEA) was used to assess the input-output efficiency of health resources, and the Mann-Whitney test was selected to explore the key factors affecting the effectiveness of DEA. ResultsAs of 2021, there are 1, 091 medical staffs in clinical department of the whole hospital, 2,084 beds, 1, 094,387 outpatients, and 81, 417 persons, average length of hospitalization (12.35±2.85) days. Average comprehensive technical efficiency of clinical department of the hospital is 0.810 from 2016 to 2021, pure technical efficiency is 0.908, scale efficiency is 0.886, DEA is effective in internal medicine group, pediatric group and the other groups, and DEA was ineffective in the other groups; the comparisons of the input-output efficiency of health resources between effective DEA group and ineffective DEA group showed that the statistical difference existed in the other items between both groups (P<0.05), except for the number of the beds (P>0.05). ConclusionOn the whole, pure technical efficiency of the hospital is relatively high, while comprehensive technical efficiency of the hospital is insufficient due to low level of resource utilization, clinical departments with valid DEA should appropriately expand input and output through scientific calculation, the ones with invalid DEA should reasonably adjust the allocation of medical and health resources, introduce discipline characteristic technology and increase the output of health resources, forming an optimal production model to realize the common and efficient development of hospitals and departments.
