摘要:
目的观察中药穴位敷贴辅助治疗气阴两虚夹瘀型老年糖尿病肾病（diabetic kidney disease，DKD）的疗效。 方法将110例老年2型DKD（Ⅲ期、Ⅳ期）患者采用随机数字表法分为对照组和观察组各55例。对照组在常规治疗基础上口服氯沙坦钾片；观察组在对照组基础上予中药穴位敷贴辅助治疗。治疗8周后观察两组患者血糖水平、肾功能、气阴两虚夹瘀证症状评分及临床疗效。 结果两组治疗后气阴两虚夹瘀证症状评分及空腹血糖(fasting blood glucose，FPG)、餐后2h血糖(2-hour postprandial blood glucose，2h PG)、糖化血红蛋白(glycosylated hemoglobin，HbAlc)均低于治疗前（P＜0.01），观察组低于对照组（P＜0.01）。治疗后两组患者24 h尿蛋白定量、尿白蛋白/肌酐比、血尿素氮、血肌酐低于治疗前，肾小球滤过率高于治疗前（P＜0.01），观察组改善更明显（P＜0.01）；观察组总有效率高于对照组（P＜0.05）。 结论在西医常规治疗基础上中药穴位敷贴能有效抑制老年DKD患者血糖水平，改善患者肾功能和中医证候。
Abstract:
ObjectiveTo observe clinical effects of acupoint application of Chinese medicine in the treatment of senile diabetic kidney disease（DKD） of Qi and Yin deficiency intermingled with blood stasis pattern as adjunctive therapy. MethodsA total of 110 senile patients suffering type two DKD (Ⅲ and Ⅳ stages) were allocated to the control group and the observation group using random number table method, with 55 cases in each group. The control group orally took losartan potassium tables on the foundation of conventional therapy; the observation group accepted acupoint application of Chinese medicine based on the therapy the control group received. To observe the levels of blood glucose, renal function, symptom scores of Qi and Yin deficiency intermingled with blood stasis pattern and curative effects after eight weeks of the treatment. ResultsAfter the treatment, symptom scores of Qi and Yin deficiency intermingled with blood stasis pattern, FPG, 2hPG, HbAlc of both groups were lower than these before the treatment (P<0.01), the observation group was lower than the control group (P<0.01). After the treatment, 24 h urinary protein quantification, urine albumin/creatinine ratio, blood urea nitrogen and serum creatinine of both groups were lower than these before the treatment, glomerular filtration rate higher than that before the treatment (P<0.01), the improvements of the observation group were more noticeable (P<0.01); total effective rate of the observation group was higher than that of the control group (P<0.05). ConclusionOn the foundation of conventional therapy of Western medicine, acupoint application of Chinese medicine could effectively inhibit the levels of blood glucose, improve renal function and TCM syndrome.
