摘要:
目的观察分清泻浊腹部推拿配合传统推拿治疗痰湿中阻椎动脉型颈椎病（cervical spondylosis of vertebral artery type，CSA）的临床疗效。 方法选取82例痰湿中阻型CSA患者，根据入院顺序将患者交替分为观察组和对照组，每组41例。对照组予以传统推拿治疗，观察组予以分清泻浊腹部推拿配合传统推拿治疗，均持续治疗2周。比较两组临床疗效和治疗前后症状积分、椎动脉型颈椎病功能评定量表（functional scale for CSA，FS-CSA）、颈椎功能障碍指数量表（neck disability index，NDI）、颈肩疼痛评分、椎动脉血流动力学参数和血清神经元特异性烯醇化酶（neuron-specific enolase，NSE）、内皮素（endothelin，ET）、超氧化物歧化酶（superoxide dismutase，SOD）水平变化情况及不良事件发生情况。 结果总有效率观察组为90.24%（37/41）高于对照组的73.17%（30/41）（P＜0.05）；治疗后观察组症状积分及FS-CSA评分、NDI评分、颈肩疼痛评分均低于对照组（P＜0.05）；观察组椎动脉平均血流速度、血管收缩期血流速度、舒张期血流速度均高于对照组（P＜0.05）；观察组血清NSE、ET、SOD平均低于对照组（P＜0.05）；观察组不良事件发生率差异无统计学意义（P＞0.05）。 结论分清泻浊腹部推拿配合传统推拿可提高痰湿中阻型CSA患者临床疗效，改善椎动脉血流动力和颈椎功能障碍，且安全性好。
Abstract:
ObjectiveTo observe clinical effects of abdominal massage of "ascending the clear and descen-ding the turbid" and traditional massage in the treatment of cervical spondylosis of vertebral artery type (CSA) of phlegm dampness obstruction pattern. MethodsAll 82 patients were selected, alternately divided into the observa-tion group and the control group according to the admission consequence with 41 cases in each group. The control group was treated with traditional massage, and the observation group received abdominal massage of "ascending the clear and descending the turbid" and traditional massage, both groups were treated for two weeks consecutively. To compare clinical effects of the two groups, symptom scores, the scores of the functional scale for cervical spondylo-sis of vertebral artery type, the scores of neck disability index (NDI), visual analogue scores (VAS) of neck and shoulder, hemodynamic parameters of the vertebral artery, serum neuron-specific enolase (NSE), endothelin, superoxide dismutase (SOD) and the incidence of adverse reaction before and after the treatment. ResultsTotal effective rate of the observation group was 90.24%(37/41), higher than 73.17%(30/41)of the control group (P<0.05); after the treatment, symptom scores, the scores of the functional scale for cervical spondylosis of vertebral artery type, the scores of NDI and VAS of neck and shoulder of the observation group were lower than these of the control group (P<0.05); mean blood flow rate of vertebral artery, vasoconstriction velocity and diastolic blood flow velocity of the observation group were higher than these of the control group (P<0.05); the levels of serum NSE, endothelin, SOD of the observation group were lower than these of the control group (P<0.05); the difference had no statistical meaning in the incidences of adverse reaction in the observation group (P>0.05). ConclusionAbdominal massage of "ascending the clear and descending the turbid" and traditional massage in the treatment of cervical spondylosis of vertebral artery type of phlegm dampness obstruction pattern could improve clinical effects, improve vertebral artery hemodynamics and neck disability with good safety.
