摘要:: 目的探讨元胡止痛滴丸术前口服对腹腔镜下子宫切除术患者的镇痛效果及其对血清5-羟色胺（5-hydroxytryptamine，5-HT）、前列腺素E₂（prostaglandin E₂，PGE₂）水平的影响。方法将116例行腹腔镜下子宫切除术患者随机分为对照组和研究组各58例。研究组患者术前2 h口服元胡止痛滴丸，对照组术前不予元胡止痛滴丸。观察两组患者术后6、12、24 h的Ramsay镇静评分和视觉模拟量表（visual analogue scale，VAS）评分、手术时间、清醒时间、呼吸恢复时间及体温恢复时间，检测两组患者术前及术后12 h血清5-HT、PGE₂水平，观察两组患者不良反应发生情况。结果术后6 h的Ramsay镇静评分研究组高于对照组（P＜0.05），术后12 h及24 h的Ramsay镇静评分两组比较差异均无统计学意义（P＞0.05）；术后6、12、24 h的VAS评分研究组低于对照组（P＜0.05）；两组患者手术时间、清醒时间、呼吸恢复时间及体温恢复时间比较差异均无统计学意义（P＞0.05）；治疗组患者术后12 h血清5-HT、PGE₂水平高于术前及对照组术后（P＜0.05）；两组患者皮肤瘙痒、呼吸抑制、恶心呕吐及头晕等不良反应发生率比较差异均无统计学意义（P＞0.05）。结论元胡止痛滴丸术前口服可以有效减轻腹腔镜下子宫切除术患者术后早期疼痛，调节血清5-HT、PGE₂水平，镇痛效果较好，而且安全可靠。

Abstract:: ObjectiveTo discuss the analgesic effects of Yuanhu (Corydalis rhizoma) pain-relieving dripping pills before the operation on patients undergoing laparoscopic hysterectomy and its influences on the levels of 5-HT and PGE₂. MethodsA total of 116 patients undergoing laparoscopic hysterectomy were randomly divided into a control group (58 cases) and a study group (58 cases). The patients in the study group took the pills two hours before the surgery, while and the ones in the control group remained unhandled before the operation. To observe the scores of Ramsay sedation scale (RSS) and visual analogue scale (VAS), operation time, resuscitation time, the recovery time of spontaneous breathing and body temperature at 6, 12 and 24 hours after the surgery, to detect the levels of 5-HT and PGE₂ before the operation and at 12 hours after the operation in the two groups, and to observe the incidences of adverse reactions in the two groups. ResultsRSS scores at 6h after the operation of the study group were higher than these of the control group (P<0.05), the difference had no statistical meaning in RSS scores at 12 and 24 h after the operation between both groups (P>0.05); VAS scores at 6, 12 and 24 hours after the surgery of the study group were lower than these of the control group (P<0.05); the difference was insignificant in the operation time, resuscitation time, the recovery time of spontaneous breathing and body temperature between the two groups (P>0.05); the levels of 5-HT and PGE₂ in the study group at 12 hours after the surgery were higher than these before the operation and these in the control group after the operation (P<0.05); the difference showed no statistical meaning in the incidences of adverse reactions such as itching, respiratory depression, nausea and vomiting, vertigo between the two groups (P>0.05). ConclusionOral administration of Yuanhu pain-relieving dripping pills before the operation could effectively relieve early pain in patients after the surgery, regulate the levels of 5-HT and PGE₂ with better analgesic effects, which is safe and reliable.
