摘要:
基于中医理论探讨强直性脊柱炎（ankylosing spondylitis，AS）与经筋学说的内在联系，提出其核心病机为督脉与膀胱经循行区域的“筋结”形成；该病理演变契合《伤寒论》外邪由表入里（皮毛→血脉→经筋）的传变规律；田氏外治法根据此机制创新性构建“三位一体”治疗体系：刮痧-指压（解皮毛层筋结），刺络-拔罐（通血脉层瘀阻），穴位埋线（松经筋层僵结）；田氏外治法通过“找结-现结-解结”诊疗路径，实现《黄帝内经》“拔刺、雪污、解结、决闭”的治疗目标。而通过分层解结策略干预AS病理进程的逻辑框架，可以揭示田氏外治法以经筋理论为指导，以理论-病理-治疗三者互为支撑的闭环关系。
Abstract:
Internal relationship between ankylosing spondylitis (AS) and the sinew channel theory is discussed based on TCM theory, and its core pathogenesis lies in the formation of "sinew nodules" in the region of governor meridian and the bladder meridian running through; the pathological progression agrees with the rules of transmission that external evils changed from the exterior to the interior (skin→blood vessels→muscles) in Shanghan Lun (Treatise on Febrile Diseases); Tian’s external treatment innovatively establish therapeutic system of "three in one": scraping-finger pressure (releasing the knots of tendons in the skin), blood-letting puncture-cupping (unobstructing the blood stasis in the blood vessels) and catgut implantation at acupoint (debonding the knots in the muscles); diagnostic and therapeutic approach of "exploring-visualizing-releasing" facilitates the realization of therapeutic goals of "pulling out the thorn-removing the pollution-untangling the knot-unobstruc-ting" in Huangdi Neijing (The Yellow Emperor’s Classic of Medicine). The logical framework of hierarchical releasing strategy interven-tion on pathological process of AS shows us the closed loop relationship of Tian’s external treatment guided by the theory of muscle between theory-pathology-treatment.
