摘要:
目的观察中医辨证穴位贴敷配合火龙罐综合灸在医院获得性肺炎患儿中的应用效果。 方法将92例医院获得性肺炎患儿随机分为常规组和观察组，每组46例。常规组予常规西医治疗，观察组在常规组基础上予中医辨证穴位贴敷配合火龙罐综合灸治疗。连续治疗16天后比较两组患者临床疗效、中医证候积分（发热、胸痛、咳嗽、痰黄稠）、肺功能指标［最大吸气压（maximum inspiratory pressure，PImax）、最大呼气压（maximum expiratory pressure，PEmax）、最大呼气中段流量（maximum mid-expiratory flow，MMF）、峰流速（peak expiratory flow，PEF）］、血清炎症因子水平［肿瘤坏死因子α（tumor necrosis factor α，TNF-α）、白细胞介素6（interleukin-6，IL-6）、IL-3］以及不良反应发生情况。 结果观察组总有效率［93.48%（43/46）］高于常规组［78.26%（36/46）］（P＜0.05）；两组各中医证候积分均降低（P＜0.05），观察组低于常规组（P＜0.05）；两组患者肺功能指标均升高（P＜0.05），观察组高于常规组（P＜0.05）；两组患者血清炎症因子水平均降低（P＜0.05），观察组低于常规组（P＜0.05）；两组治疗期间总不良反应发生率比较，差异无统计意义（P＞0.05）。 结论中医辨证穴位贴敷配合火龙罐综合灸治疗医院获得性肺炎临床疗效明确，能有效提高患儿肺功能，降低机体炎性反应，且安全可靠。
Abstract:
ObjectiveTo observe the application of acupoint plaster application based on syndrome differentiation and comprehensive moxibustion with fire-dragon cupping in children with hospital-acquired pneumonia (HAP). MethodsAll 92 children with HAP were randomized to the conventional group and the observation group with 46 cases in each. Conventional Western medicine was given to the conventional group while the observation group was treated with acupoint plaster application based on syndrome differentiation and comprehensive moxibustion with fire-dragon cupping. To compare clinical effects, TCM syndrome score (fever, chest pain, cough, and yellow-thick phlegm), and the indexes of lung function including PImax, PEmax and MMF, PEF, the levels of serum inflammatory factors such as TNF-α, IL-6 and IL-3, together with the incidence of adverse reaction after 16 consecutive days of the treatment between the two groups. ResultsTotal effective rate of the observation group was [93.48%(43/46)], higher than [78.26%(36/46)] of the conventional group (P<0.05); TCM syndrome scores were lowered in the two groups (P<0.05), the observation group was lower than the conventional group (P<0.05); the increase in the indexes of lung function was found in the two groups (P<0.05), the observation group was higher than the conventional group (P<0.05); the decrease in the levels of the inflammatory factors was found in the two groups (P<0.05), the observation group was lower than the conventional group (P<0.05); and the difference had no statistical meaning in the incidence of adverse reaction during therapeutic period between the two groups (P>0.05). ConclusionAcupoint plaster application based on syndrome differentiation combined with comprehensive moxibustion with fire-dragon cupping demonstrates efficacy in treating pediatric HAP, and it could effectively improve lung function, and reduce inflammatory response of human body, which is safe and reliable.
