摘要:
目的探讨原发性闭角型青光眼（primary angle-closure glaucoma，PACG）视神经损害应用西药联合明目汤的治疗方法，并分析其治疗效果。 方法选择120例PACG患者，采用随机数字表法分为对照组和观察组，每组60例。对照组予常规西药治疗，观察组在对照组的基础上予明目汤治疗，两组均连续治疗4周。比较两组临床疗效及治疗前后视野检查结果［视野缺损程度（mean deviation，MD）、中心视野平均光敏度（mean sensitivity，MS）］、眼压、视网膜中央动脉血流指标［阻力指数（resistive index，RI）、收缩期血流峰值速度（peak systolic velocity，PSV）、舒张末期血流速度（end-diastolic velocity，EDV）］、视神经功能指标［潜伏期、振幅的图形视觉诱发电位（pattern visual evoked potential，P-VEP）］的变化。 结果治疗后，观察组治疗有效率［96.67（58/60）］高于对照组［83.33（50/60）］（P＜0.05）；两组患者MD、眼压、RI、潜伏期P-VEP均较治疗前降低（P＜0.05），MS、PSV、EDV、振幅P-VEP均升高（P＜0.05），且观察组改善程度大于对照组（P＜0.05）。 结论应用西药联合明目汤治疗PACG视神经损害，可有效提高临床疗效，降低眼压，改善视网膜中央动脉血流，从而促进视神经功能恢复。
Abstract:
ObjectiveTo explore the therapeutic methods of combining Western medicine with eyesight-improving decoction for optic nerve damage in primary angle-closure glaucoma (PACG) patients, and to analyze its therapeutic effects. MethodsA total of 120 PACG patients were selected and allocated to the control group and the observation group using random number table method with 60 cases in each. The control group was treated with conventional Western medicine while eyesight-improving decoction was administered to the observation group, both groups were treated for four consecutive weeks. To compare clinical effects, results of visual inspection before and after the treatment (MD, MS), intraocular tension and blood flow indexes of central retinal artery (CRA) (RI, PSV and EDV) and the indexes of optic nerve function (incubation period, P-VEP) in the two groups. ResultsAfter the treatment, the effective rate of the observation group was [96.67%(58/60)], higher than [83.33%(50/60)] of the control group (P<0.05); MD, intraocular tension, RI and P-VEP of incubation period were lower in the two groups than before the treatment (P<0.05), MS, PSV and EDV, alongside amplitude P-VEP were elevated (P<0.05), and the improvements of the observation group were larger than these of the control group (P<0.05). ConclusionCombining Western medicine with eyesight-improving decoction for optic nerve damage in PACG patients could effectively boost clinical effects, reduce intraocular tension, and improve blood flow indexes of CRA, thereby promoting the recovery of optic nerve function.
