摘要:
基于“不通则痛”理论探讨桂枝茯苓丸在慢性前列腺炎（chronic prostatitis，CP）中缓解疼痛的作用机制。指出湿热、气滞、瘀血是CP“不通则痛”的重要病机；炎症反应加剧、血液流速降低、盆底肌肉异常收缩、氧化应激反应是CP疼痛的重要发病机制；中医学认为桂枝茯苓丸具有活血化瘀、消癥止痛功效，现代药理研究表明其可通过镇痛、抗炎、抗氧化、调节肌肉收缩、改善血液流变等作用缓解疼痛；临床研究显示，以“不通则痛”理论为指导，单独应用桂枝茯苓丸、桂枝茯苓丸联合清热散结方剂或中医外治疗法，均能缓解CP患者盆腔区域的疼痛不适。
Abstract:
The paper discusses the mechanism of Guizhi Fuling pills in relieving pain in chronic prostatitis (CP) based on the theory of "pain due to obstruction". It is proposed that damp-heat, Qi stagnation, and blood stasis are the key pathogenesis of "pain due to obstruction" in CP. Furthermore, increased inflammatory response, decreased blood flow velocity, abnormal pelvic floor muscle contraction, and oxidative stress are important mechanisms underlying pain in CP. Traditional Chinese Medicine holds that Guizhi Fuling pills have the effects of promoting blood circulation, removing blood stasis, resolving abdominal masses, and alleviating pain. Modern pharmacological studies have shown that it can relieve pain through analgesic, anti-inflammatory, antioxidant, muscle contraction-regulating, and hemorheology-improving actions. Clinical studies have shown that guided by the theory of "pain due to obstruction", the use of Guizhi Fuling pills alone, or in combination with heat-cleating and stasis-resolving formulas or external therapies of Chinese medicine, can alleviate pelvic pain and discomfort in CP patients.
