摘要:
目的：观察防风荆芥甘草汤坐浴治疗会阴伤口愈合不良的疗效。方法：将80例会阴伤口愈合不良的产妇随机分为2组。实验组40例，用防风荆芥甘草汤水煎取1 000 mL，坐浴，20 min/次；对照组40例以伤口清洁换药3～5日后，在局麻下将伤口清创并行二次缝合，同时静滴抗菌素3日。结果：治疗后会阴伤口疼痛实验组较对照组减轻(P＜0.05)。治疗第3天Ⅳ级疼痛实验组0例，对照组4例，2组相比差异有统计学意义(P＜0.05)。治疗后会阴伤口愈合情况实验组明显好于对照组(P＜0.05)。实验组愈合时间短于对照组，2组相比差异有统计学意义(P＜0.05)。实验组住院时间最短3天，最长5天，平均4天；对照组住院时间最短8天，最长12天，平均10天。实验组住院时间短于对照组(P＜0.05)。2组用药期间均无不良反应，但实验组仍可哺乳。结论：应用防风荆芥甘草汤坐浴治疗会阴伤口愈合不良临床疗效显著，避免了二次缝合伤口，并可缩短住院时间及愈合时间。
Abstract:
Objective: To observe effects of FangFeng JingJie GanCaoTang in treating abnormal healing of perineal wounds. Method: Eighty puerperants with poor healing of perineal wounds were allocated into experimental group and control group randomly. Forty cases in experimental group accepted hip bath of FangFeng JingJie GanCaoTang, 1 000 mL, 20 minutes each time; after cleaned and changed dressing for three to five days, control group received surgical debridement, second suture under local anaesthesia and intravenous dripping of antibiotics for three days. Result: Pain of perineal wounds in experimental group alleviated obviously after the treatment compared with control group (P＜0.05). At the third day of treatment, there was no pain of grade four in experimental group, less than four cases in control group with remarkable difference (P＜0.05). Healing conditions of perineal wounds after the treatment in experimental group were better than that in control group (P＜0.05). Healing times of experimental group were shorter than that of control group with remarkable difference (P＜0.05). The shortest stay in experimental group was three days, the longest five days and the average four days, while the shortest in control group was eight days, the longest twelve days and the average ten days, and the length of hospital stay in experimental stay was shorter than that in control group(P＜0.05). During medication period both groups were without adverse reaction and experimental group can still nurse the babies. Conclusion: Hip bath of FangFeng JingJie GanCaoTang is effective in treating poor healing of perineal wounds obviously, which can avoid the second suture, shorten the length of hospital stay and healing period.
