摘要:
目的：观察丙氨酰谷氨酰胺联合血必净注射液治疗重症急性胰腺炎的临床疗效。方法：将78例重症急性胰腺炎患者随机分为治疗组38例、对照组38例，对照组给予禁食、胃肠减压，应用生长抑素、质子泵抑制剂、抗生素等常规治疗。治疗组在常规治疗的基础上以丙氨酰谷氨酰胺20 g+5%葡萄糖注射液500 mL，联合血必净注射液50 mL+生理盐水100 mL，静脉滴注，1次/d，连用7天为1个疗程。观察用药前后肠功能、临床症状消失时间、体温恢复正常时间、白细胞计数及血淀粉酶恢复正常时间、平均住院时间，CRP水平的变化、并发症的发生率。结果：临床症状缓解时间、体温、血淀粉酶、白细胞计数恢复正常时间及平均住院时间治疗组均短于对照组（P＜0.05）；治疗后2、3、7天,血清CRP水平治疗组较对照组下降显著（P＜0.05）。并发症发生率治疗组低于对照组（P＜0.05）。结论：丙氨酰谷氨酰胺联合血必净注射液治疗重症急性胰腺炎疗效显著。
Abstract:
Objective: To survey therapeutic effects of alanyl-glutamine and XueBiJing injection in treating severe acute pancreatitis (SAP). Methods: Altogether 78 SAP patients were randomly divided into the treatment group and the control group, 38 cases each group, the control group were administered with fasting, gastrointestinal decompression, routine treatment including somatostatin, proton pump inhibitor, antibiotics. On the foundation of the former therapy, the treatment group received intravenous dripping of alanyl-glutamine, 20 g, into 5% glucose, 500mL, and XueBiJing injection, 50 mL, into 9% physiological saline, 100 mL, once per day, seven days were one course of the treatment. Intestinal function, disappearing time of clinical symptoms, the time of body temperature return to normal, the time of white blood cells counting and hemodiastase retrun to normal, average hospitalization time, the levels of CRP and the incidence of the complications were observed. Results: Relieving time of clinical symptom, the time of body temperature, hemodiastase and WBC counting return to normal and average hospitalization time of the treatment group were shorter than these of the control group (P＜0.05); after treating for two, three and seven days, the levels of serum CRP in the treatment group were lowered more obviously than these of the control group (P＜0.05). The treatment group was lower than the control group in the incidences of the complications (P＜0.05). Conclusion: Alanyl-glutamine and XueBiJing injection are effective in treating SAP.
