摘要:
目的：通过护理干预流程管理三联疗法治疗寻常型银屑病的顺序，探讨治疗、护理寻常型银屑病最佳流程管理方案。方法：将寻常型银屑病患者180例随机分为A1、A2、B1、B2、C1、C2 6组(均给予NB-UVB照射隔日1次、局部外用药封包每天1次、中药药浴每天1次)。干预内容：改变三联疗法的先后顺序，即：A1依次顺序为窄波UVB治疗、局部外用药封包治疗、中药药浴治疗；A2依次顺序为窄波UVB治疗、中药药浴治疗、局部外用药封包治疗；B1依次顺序为中药药浴治疗、窄波UVB治疗、局部外用药封包治疗；B2依次顺序为中药药浴治疗、局部外用药封包治疗、窄波UVB治疗；C1依次顺序为局部外用药封包治疗、窄波UVB治疗、中药药浴治疗；C2依次顺序为局部外用药封包治疗、中药药浴治疗、窄波UVB治疗。通过6组患者治疗前、治疗第2周、第3周及第4周时PASI评分值观察疗效，通过治疗期间患者出现不良反应观察其安全性，判断最优流程管理。结果：C2组治疗第2周、第3周及第4周PASI评分差均值与其他各组比较有明显增加，且副作用最低，差异有统计学意义(P＜0.05)。结论：三联疗法治疗寻常型银屑病最有效的护理干预顺序为先给予局部外用药封包治疗，再给予中药药浴治疗，最后予窄波UVB治疗。
Abstract:
Objective: To explore the best procedure management plan of treating and nursing psoriasis vulgaris (PV) by intervening the orders of triple therapy in treating PV through nursing intervention. Methods: All 180 PV patients were randomized into six groups: A1, A2, B1, B2, C1 and C2 (they all accepted NB-UVB radiation, once every other day, drug package for local and external use, once each day, and herbal bath, once each day). The intervention contents: changing the order of triple therapy, that is: the patients in the group A1 were treated by NB-UVB, drug package for local and external use and herbal bath; A2: NB-UVB, herbal bath and drug package for local and external use; B1: herbal bath, NB-UVB and drug package for local and external use; B2: herbal bath, drug package for local and external use and NB-UVB; C1: drug package for local and external use, NB-UVB and herbal bath; C2: drug package for local and external use, herbal bath and NB-UVB. The safety of the therapy was assessed by observing the adverse reaction happened to the patients during therapeutic periods, clinical effects were observed by PASI scales before treating, at the second, third and forth week in six groups, the best procedure management was judged by comparing statistical study. Results: Rank sum tests were performed in different groups, and the groups were compared in pairs, mean PASI scales of C2 group at the second, third and forth week increased notably when they were compared with different groups, with fewer side effects, and the differences had statistical meaning (P＜0.05). Conclusion: The most effective nursing intervention orders for PV patients treated by triple therapy are drug package for local and external use firstly, herbal bath and NB-UVB finally.
