摘要:
目的：观察中医护理干预预防髋关节置换术后深静脉血栓的临床疗效。方法：将126例患者随机分为观察组、对照组各63例。对照组给予常规护理，观察组在对照组护理基础上给予中医护理干预。观察2组术前、术后第1日、术后第3日、术后1周、术后2周各时间点下肢静脉血栓及下肢肿胀情况、肢体温度、肢体Human征及Nenhof征。结果：观察组发生下肢静脉血栓0例，发生下肢肿胀2例(3.17%)；对照组发生下肢静脉血栓8例(12.7% )，发生下肢肿胀10例(15.9%)。下肢深静脉血栓及下肢肿胀发生率2组比较，差异有统计学意义(P＜0.05)。患肢体表温度2组术前比较，差异无统计学意义(P＞0.05)；术后各相同时间点观察组均低于对照组，差异有统计学意义(P＜0.05)。观察组Human征阳性12例(19.04%)，Nenhof征阳性10例(15.69%)；对照组Human征阳性36例(57.14%)，Nenhof征阳性32例(50.79%)。Human征阳性率及Nenhof征阳性率2组比较，差异均有统计学意义(P＜0.05)。结论：早期进行中医护理干预能有效降低髋关节置换术后并发症的发生率，有利于恢复髋关节功能。
Abstract:
Objective: To observe curative effects of TCM nursing intervention in preventing deep venous thrombosis of the patients underwent total hip replacement. Methods: All 126 patients were randomized into the observation group and the control group, 63 cases each group. The control group adopted routine care, and the observation group TCM nursing intervention on the foundation of nursing the control group accepted. Venous thrombosis of lower limbs and the swelling of lower limbs, limb temperature, limb Human sign and Nenhof sign in both groups were observed before the operation, at the first day, the third day, the first week and the second week after the operation. Results: There was zero case suffering from deep venous thrombosis of lower limb, and two cases with lower limb swelling(3.17%) in the observation group; there were eight cases with deep venous thrombosis of lower limb(12.7%), and ten cases with lower limb swelling (15.9%) in the control group. The difference had statistical meaning in the comparison of the incidences of lower limb deep venous thrombosis and swelling(P＜0.05). The difference had no statistical meaning in the comparison of the affected limbs temperature before the surgery between both groups(P＞0.05); the observation group was lower than the control group in different times after the operation, and the difference had statistical meaning (P＜0.05). There were 12 cases with positive Human sign(19.04%) and ten cases with positive Nenhof sign (15.69%); 36 cases with positive Human sign(57.14%) and 32 cases with positive Nenhof sign(50.79%). The difference had statistical meaning in the comparison of positive incidences of Human sign and Nenhof sign between both groups(P＜0.05). Conclusion: Early TCM nursing intervention could effectively decrease the incidence of the complications after the surgery of hip replacement, which is helpful to hip joint function recovery.
