摘要:
目的：观察穴位贴敷治疗慢性阻塞性肺疾病稳定期的临床疗效及其对外周血T细胞亚群的影响。方法：将慢性阻塞性肺疾病稳定期患者76例按照随机数字表法分为治疗组和对照组各38例。对照组给予西医常规综合治疗，治疗组在此基础上给予中药穴位贴敷治疗，2组疗程均为4周。观察2组患者治疗前后中医临床症状评分、CAT评分及T细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)水平变化情况。结果：治疗后2组中医临床症状评分均低于治疗前，治疗组治疗后症状积分低于对照组(P＜0.05)。显效率、有效率治疗组分别为53.68%，86.84%，对照组分别为21.05%，71.05%，治疗组优于对照组(P＜0.05)。治疗后2组患者CAT评分均低于治疗前，且治疗组治疗后CAT评分低于对照组(P＜0.05)。治疗后2组CD3+、CD4+以及CD4+/CD8+水平较治疗前明显上升，CD8+降低，治疗组改善优于对照组(P＜0.05)。结论：中药穴位贴敷治疗慢性阻塞性肺疾病稳定期疗效确切，作用机制可能与通过调节T细胞亚群，进而调节免疫机能有关。
Abstract:
Objective: To observe clinical effects of acupoint application in treating chronic obstructive pulmonary disease (COPD) at stationary stage and its effects on T lymphocytes in peripheral blood. Methods: All 76 patients with COPD at stationary stage were randomized into the treatment group and the control group according to random number table method, 38 cases each group. The control group accepted conventional treatment of western medicine, and the treatment group were given herbal acupoint application, the course of the treatment was four weeks for both groups. TCM clinical symptom scales, CAT scales and the levels of T lymphocytes (CD3+, CD4+, CD8+, CD4+/CD8+) of both groups were observed before and after treating. Results: TCM clinical symptom scales of both groups after treating were lower than these before treating, and the treatment group was lower than the control group in symptom scales after treating (P＜0.05). The markedly effective rate and the effective rate of the treatment group were 53.68% and 86.84% respectively, superior to 21.05% and 71.05% of the control group (P＜0.05). CAT scales of both groups after treating were lower than these before treating, and symptom scales of the treatment group after treating were lower than these of the control group (P＜0.05). After treating, the levels of CD3+, CD4+ and CD4+/CD8+ raised compared with these before treating notably, the levels of CD8+ decreased, the treatment group was superior to the control group (P＜0.05). Conclusion: Herbal acupoint application in treating COPD at stationary stage could obtain definite effects, and its mechanism might be related to regulating immune function through regulating T lymphocytes.
