摘要:: 目的：探讨中西医结合护理干预对不完全性脊髓损伤后神经源性膀胱的护理体会。方法：将不完全性脊髓损伤神经源性膀胱患者84例，采用随机数字表法分为观察组和对照组各42例。对照组予常规护理，保持导尿配合膀胱功能训练，观察组予间歇性清洁导尿，膀胱功能训练，计划饮水，配合下腹部中药局部热敷及点穴疗法。比较2组平均排尿频率、残余尿量、膀胱容量、泌尿感染情况、导尿持续时间及临床有效率。结果：2组在治疗前、治疗2周时，平均排尿频数、残余尿量、膀胱容量方面比较差异均无统计学意义（P＞0.05）；在治疗1月时，2组此3项指标比较差异具有统计学意义（P＜0.05）。泌尿系感染观察组4例(9.52%)，对照组17例(40.48%)，差异具有统计学意义( χ2=10.730,P=0.001)。观察组导尿持续时间少于对照组，差异有统计学意义（P＜0.05）。临床总有效率观察组为85.71%，高于对照组的66.71%，2组比较差异有统计学意义（ χ2=4.2，P＜0.05）。结论：中西医结合护理方法能够较为有效地促进不完全脊髓损伤神经源性膀胱功能的恢复，降低并发症的发生率，提高患者生活质量。

Abstract:: Objective: To discuss the integrated traditional Chinese and western medicine nursing experience for the patients with neurogenic bladder after incomplete spinal cord injury. Methods: All 84 patients were separated into the observation group and the control group according to random number table, 42 cases each group. The control group accepted routine care, maintained catheterization and bladder function training, and the observation group were given clean intermittent catheterization, bladder function training, planned drinking water, and local herbal hot compress in the lower abdomen and point percussion therapy. Average urination frequencies, residual urine volumes, bladder volumes, the conditions of urinary infection, the duration of catheterization and clinical effective rates of both groups were compared. Results: The difference had no statistical meaning in the comparisons of average urination frequencies, residual urine volumes and bladder volumes in both groups between before treating and in two weeks after treating(P＞0.05); the difference had statistical meaning in the comparison of three indexes between both groups in one month after treating(P＜0.05). There were four cases infected in the observation group (9.52%), and 17 cases with urinary system infection in the control group(40.48%), and the difference had statistical meaning( χ2=10.730, P=0.001). The observation group was less than the control group in the duration time of catheterization, and the difference had statistical meaning (P＜0.05). Clinical total effective rate of the observation group was 85.71%, higher than 66.71% of the control group, and the difference showed statistical meaning( χ2=4.2, P＜0.05). Conclusion: Integrated traditional Chinese and western medicine nursing for the patients with neurogenic bladder after incomplete spinal cord injury could promote the recovery of bladder function effectively, decrease the incidence of the complications, and improve life quality of the patients.
