摘要:
目的：系统评价补肾中药联合达英-35治疗多囊卵巢综合征（PCOS）的疗效及安全性。方法：计算机检索相关中英文数据库关于补肾中药联合达英-35治疗PCOS的随机对照试验（RCT），各数据库检索时间均由建库至2017年2月。对符合纳入和排除标准的文献进行数据提取及方法学质量评价，应用RevMan 5.3软件作Meta分析。结果：共纳入文献22篇，1676例患者。Meta分析结果显示：补肾中药联合达英-35与单用达英-35组对比，联合用药组总有效率更高［OR=4.22，95%CI(2.86,6.23)，P＜0.00001］，雄激素［MD=-0.45，95%CI(-0.67,-0.22)，P＜0.0001］、促黄体生成素［MD=-1.84，95%CI(-1.98,-1.70)，P＜0.00001］、LH/FSH值［MD=-0.25，95%CI(-0.44,-0.06)，P=0.009］等性激素水平改善均优于单用西药组，并能明显提高妊娠率［OR=3.34，95% CI(2.23,5.02)，P＜0.00001］，且未出现明显不良反应。结论：现有证据显示补肾中药联合达英-35治疗PCOS疗效显著，能明显改善性激素水平，提高总有效率及妊娠率，不良反应少，可供临床参考与应用。
Abstract:
Objective: To systematically assess clinical effects and the safety of kidney-tonifying herbs combined with Diane-35 in the treatment for polycystic ovarian syndrome (PCOS). Methods: Randomized controlled trials (RCTs) of kidney-tonifying herbs combined with Diane-35 in treating PCOS were retrieved by the computer in Chinese-English database, and the retrieval time was from the time of database building to February, 2017. The data exracting and methodological quality assessment were performed in the literature which met the inclusion and exclusion criteria, and meta analysis was performed by using RevMan software 5.3. Results: Altogether 22 papers included and 1676 patients involved. Meta analysis results showed that: when kidney-tonifying herbs combined with Diane-35 were compared with only Diane-35, total effective rate of the combination medication was higher [OR=4.22, 95%CI (2.86, 6.23), P＜0.00001], the improvements of androgen [MD=-0.45, 95%CI (-0.67, -0.22), P＜0.00001], luteinizing hormone [MD=-1.84, 95%CI (-1.98, -1.70), P＜0.00001], LH/FSH values [MD=-0.25, 95%CI (-0.44, -0.06), P=0.009] and other sex hormones were superior to these of western medicine group only, the combination drugs could raise the pregnancy rate obviously [OR=3.34, 95%CI (2.23, 5.02), P＜0.00001], and there was no obvious adverse reaction. Conclusion: The existing evidence shows that kidney-tonifying herbs combined with Diane-35 could obtain notable effects in treating PCOS, the therapy could improve the levels of sex hormone remarkably, raise total effective rate and the pregnancy rate with less adverse reaction, which could provide for clinical reference and application.
