摘要:
通过4则验案探讨王自立主任医师运脾思想治疗脾胃病的经验。“健脾先运脾，运脾必调气”为王自立主任医师临床治疗脾虚不运的主导思想，用药贵在切病，辨证准确，则药精方简而效佳，而且在药物用量上以轻为上，小剂量施治，处处保护脾胃的生发之气；药性则不可过于苦寒以免伤脾败胃，不可香燥太过而耗胃阴，不可滋腻大补以碍脾运，需补而不滞，滋而不腻，理气而不破气；运脾汤中枳壳为运脾调气之关键，寓理气于补益之中，寓调气于健胃之间，共奏健脾促运，调气和胃之效。
Abstract:
Chief physician Wang Zili′s experience of applying his unique spleen-activating thinking to spleen-stomach disease was eluciadated by four proven cases, "spleen-activating is the first step of spleen-invigorating, it is imperative to regulate Qi" was taken as the dominant idea of treating spleen deficiency and failing to transport in clinic, the important thing of the treatment was pulse-taking, if the syndrome was differentiated accurately, excellent herbs and simple prescription could obtain the best effects, furthermore, the dose of the herbs should be low, small dose could protect the developmental Qi of spleen and stomach; the nature of the herbs should not be too bitter so as not to impair the spleen and hurt the stomach, not be too fragrant and dry so as not to consume stomach-Yin, not be too greasy so as not to obstruct spleen transportation, the treatment should follow the principles of supplementing while not stagnating, moistening while not greasing, regulating Qi while not breaking Qi; ZhiQiao(fructus aurantii) was the key of activating spleen and regulating Qi in spleen-activating decoction, it can regulate Qi when tonify, adjust Qi when strengthen the stomach, and it can take effects of invigorating spleen and promoting the digestion, regulating Qi and harmonizing the stomach.
