摘要:
目的：观察针灸结合康复训练治疗脊髓损伤患者神经源性膀胱的临床疗效。方法：将脊髓损伤导致的神经源性膀胱患者90例随机分为大膀胱和小膀胱2组，均采用针灸结合康复训练治疗，4周后评估2组患者膀胱功能积分、残余尿量及膀胱容量。结果：针灸结合康复训练可减少神经源性大膀胱患者的膀胱残余尿量，增加膀胱容量，提高膀胱功能，降低膀胱功能积分；但此法对脊髓损伤导致的神经源性小膀胱疗效不佳。结论：对于脊髓损伤导致的神经源性膀胱患者尽早加入膀胱康复训练及针灸治疗，对于促进患者神经源性膀胱功能的恢复起决定性作用，并且可避免小膀胱的出现；而对于神经源性小膀胱患者建议针刺治疗，以刺激轻、周期长为主，并配合膀胱康复训练或药物治疗，或许可达到理想治疗效果。
Abstract:
Objective: To observe clinical effects of acupuncture combined with rehabilitation training in treating neurogenic bladder of the patients with spinal injury. Methods: All 90 patients were randomly divided into two groups including big bladder and small bladder, and they used acupuncture combined with rehabilitation training, bladder function integral, residual urine volume (RUV) and bladder capacity of both groups were assessed in four weeks. Results: Acupuncture combined with rehabilitation training could reduce RUV of the patients with neurogenic big bladder, increase bladder capacity, improve bladder function and lower bladder function integral; while the method was not effective in treating spinal injury-induced neurogenic small bladder. Conclusion: It’s imperative to participate in bladder rehabilitation training and acupuncture for the patients suffering spinal injury-induced neurogenic bladder, which could promote functional recovery of the patients, avoid small bladder; for the patients with neurogenic small bladder, acupuncture is suggested since it could stimulate lightly with a long cycle, acupuncture combined with bladder rehabilitation training or medicine might attain perfect therapeutic effects.
