摘要:
目的：探讨滋阴补气祛瘀饮联合西药对抗心磷脂抗体(anticardiolipin antibody,ACA)阳性反复自然流产患者疗效及性激素水平的影响。方法：将ACA阳性反复自然流产患者80例按随机数字表法分为对照组和观察组各40例，对照组患者口服阿司匹林100 mg，每日1次；肌肉注射绒毛膜促性腺性激素每次2 000 U，每日1次，观察组在对照组治疗的基础上加用滋阴补气祛瘀饮，治疗3个月后比较两组患者近期疗效，ACA转阴率，远期妊娠率，治疗前后中医证候积分、子宫动脉血流参数指标及性激素水平。结果：观察组总有效率、ACA转阴率及远期妊娠率均高于对照组(P＜0.05)；观察组治疗后中医证候评分低于对照组及治疗前(P＜0.05)；观察组治疗后子宫动脉血流收缩期峰值流速(peak systolic velocity，PSV)、舒张末期血流速度（end diastolic velocity,EDV）水平均高于对照组及治疗前(P＜0.05)，阻力指数（resistance index，RI）水平低于对照组及治疗前(P＜0.05)；观察组治疗后人绒毛膜促性腺性激素（human chorionic gonadotropin，HCG）、孕酮(progesterone，P)及雌二醇(estradiol，E2)水平均高于对照组及治疗前(P＜0.05)。结论：滋阴补气祛瘀饮联合西药可有效缓解ACA阳性反复自然流产患者临床症状，提高远期妊娠率，促进ACA转阴，改善子宫动脉血流参数，并有助于调节性激素水平。
Abstract:
Objective: To explore the effects of Yin-nourishing Qi - invigorating stagnation - eliminating drink combined with Western medicine on clinical effects and sex hormone level of the patients with positive - ACA recurrent spontaneous abortion (RSA). Methods: Eighty patients were divided into the control group and the observation group according to random number table method, 40 cases each group, the control group took aspirin orally, 100mg, once each day; muscular injection of human chorionic gonadotropin (HCG), 2 000 U each time, once each day, the observation group took Yin - nourishing Qi-invigorating stagnation-eliminating drink, to compare short-term effects, ACA negative conversion rate, long-term pregnancy rate, TCM syndrome scores before and after treating, uterine artery blood flow parameters and sex hormone level between both groups after treating for five days. Results: The observation group was higher than the control group in total effective rate, ACA negative conversion rate and long-term pregnancy rate (P＜0.05); TCM syndrome scores of the observation group after treating were lower than these of the control group and the observation group before treating (P＜0.05); the observation group after treating was higher than the control group and the observation group before treating in PSV and EDV levels (P＜0.05), lower than the control group and the observation group before treating in RI levels (P＜0.05); HCG, P and E2 levels of the observation group after treating were higher than these of the control group and the observation group after treating (P＜0.05). Conclusion: Yin-nourishing Qi-invigorating stagnation-eliminating drink combined with Western medicine could effectively relieve clinical symptoms of the patients, raise long-term pregnancy rate, promote ACA negative conversion and improve uterine artery blood flow parameters and it is helpful to regulate sex hormone levels.
