摘要:
目的：探讨子宫全切术后中医穴位按摩的护理干预方法及效果。方法：将子宫全切术患者100例按照护理干预方式不同分为观察组与对照组各50例。对照组实施常规护理，观察组在常规护理基础上予中医穴位按摩护理；观察2组患者疼痛缓解有效率、子宫全切术后腹胀情况、肠蠕动恢复时间与肛门排气以及子宫全切术后尿管拔除后排尿情况、患者心理状态、护理满意度。结果：疼痛缓解有效率观察组为64%，高于对照组的32%(P＜0.05)；观察组中、重度腹胀发生率为22%，低于对照组的62%(P＜0.05)；肠蠕动恢复时间与术后肛门排气时间观察组短于对照组(P＜0.05)；观察组首次排尿时间较对照组早(P＜0.05)，观察组排尿成功率为98%，高于对照组的88%(P＜0.05)，且尿潴留发生率为2%，低于对照组的14%(P＜0.05)；子宫全切术后观察组患者焦虑评分低于对照组(P＜0.05)；观察组护理满意度为94%，高于对照组的72%(P＜0.05)。结论：子宫全切术后对患者实施中医穴位按摩护理，不仅能有效缓解患者术后疼痛与腹胀情况，而且能有效缩短患者术后肠蠕动恢复时间与术后肛门排气时间，还能有效缩短患者首次排尿时间，提高患者排尿成功率，降低患者尿潴留发生率，同时能改善患者术后心理状态，提升患者的护理满意度。
Abstract:
Objective: To discuss nursing intervention methods and the effects of TCM acupoint massage after total hysterectomy (TH). Methods: One hundred patients were randomized into the observation group and the control group by different nursing intervention methods. The patients in the control group accepted conventional nursing, the observation group TCM acupoint massage nursing based on conventional care; nursing satisfaction degrees, the patients' psychological state, urination conditons after removing urinary catheter after HT, anal exhaust, the recovery time of peristalsis, the conditions of abdominal distension after HT, the effective rate of pain remission of the patients were observed in both groups. Results: The effective rate of pain remission in the observation group was 64%, higher than 32% of the control group (P＜0.05); the incidence of severe abdominal distension of the observation group was 22%, lower than 62% of the control group (P＜0.05); the observation group was shorter than the control group in the recovery time of peristalsis and postoperative anal exhaust time (P＜0.05); the observation group was earlier than the control group in initial urination time (P<0.05), urination success rate of the observation group was 98%, higher than 88% of the control group (P＜0.05), the uroschesis incidence was 2%, lower than 14% of the control group (P＜0.05), after HT, the patients of the observation group were lower than these of the control group in anxiety scores (P＜0.05), nursing satisfaction degrees of the observation group was 94%, higher than 72% of the control group (P＜0.05). Conclusion: TCM acupoint massage nursing for the patients after HT could not only relieve postoperative pain and abdominal distension of the patients effectively, but also shroten postoperative recovery time of peristalsis and anal exhaust time, effectively shorten initial urination time, raise urination success rate, decrease the uroschesis incidence, improve the patients' postoperative psychological state and raise nursing satisfaction degrees.
