摘要:
目的：总结施杞教授治疗强直性脊柱炎(AS)的临床用药规律，探索其学术思想。方法：收集2009—2015年间就诊于龙华医院施杞教授门诊且符合纳入标准的典型AS患者193例，对纳入医案中使用的药物采用SPSS 21.00软件进行频数分析和聚类分析。结果：共计使用药物209种，总频次为4 483。川芎、柴胡、熟地黄、党参、当归、炙甘草6味中药是最常用的药物，补虚药、解表药、活血化瘀药、祛风湿药、行气药、清热药、化湿药是施杞教授临证常用的药物。聚类分析显示圣愈汤是施杞教授治疗AS的核心处方，身痛逐瘀汤、当归拈痛汤是施杞教授治疗AS的基本处方。结论：益气活血化瘀法是施杞教授治疗AS的基本大法，体现了其治疗AS气血并重，扶正兼顾外邪的学术思想，在一定程度上揭示了施杞教授治疗AS的用药规律。
Abstract:
Objective: To summarize professor Shi Qi′s clinical medication rules of treating ankylosing spondylitis (AS) and explore his academic thinking. Methods: After collecting 193 cases of typical AS who visited professor Shi Qi′s outpatient, Longhua Hospital, from 2009 to 2015 and met the inclusion criteria, the herbs used in the medical cases were performed frequency analysis and cluster analysis using software SPSS21.00. Results: Altogether 209 herbs were used and total frequency was 4 483. Chuanxiong (Chuanxiong rhizoma), Chaihu (bupleuri radix), Shudihuang (Rehmanniae radix praeparata), Dangshen (codonopsis radix), Danggui (angelicae sinensis radix) and Zhigancao (glycyrrhizae radix et rhizoma praeparata cum melle) were the common herbs, the herbs of tonification, relieving the exterior, promoting blood circulation and eliminating the stasis, dispelling wind-dampness, promoting the circulation of Qi, heat-clearing, dampness-eliminating were the common ones used by professor Shi Qi. Cluster analysis demonstrated that ShengYu decoction was the core prescription used by professor Shi Qi in clinic, ShenTong ZhuYu decoction and DangGui NianTong decoction are the basic prescriptions of treating AS used by professor Shi Qi. Conclusion: Invigorating-Qi promoting blood circulation and stasis-eliminating method is the basic method of treating AS, representing professor Shi′s academic thinking of emphasizing Qi and blood, enhancing healthy Qi and eliminating external pathogenic factors, and it reveals his medication rules of treating AS to a certain degree.
