摘要:
目的：探讨补肾中药对肾阴虚型体外受精-胚胎移植(in vitro fertilization and embryo transfer，IVF-ET)患者子宫内膜容受性的影响。方法：将80例肾阴虚型IVF-ET患者采用随机数字法分为两组，其中空白组40例，观察组40例，另选取单纯输卵管因素行IVF-ET的40例患者作为对照组，对照组和空白组患者给予常规方案治疗，观察组在此基础上给予补肾中药治疗。观察比较三组患者子宫内膜厚度、容积及内膜下血流的变化，比较患者两侧子宫动脉搏动指数(pulsatility index，PI)、阻力指数(resistance index，RI)、内膜下血管化指数(vascularization index，VI)以及血流指数(flow index，FI)和血管化血流指数(vascularized blood flow index，VFI)的水平，同时比较患者着床期子宫内膜中整合素β3(αvβ3)mRNA、同源框基因10(Hox10)mRNA、基质金属蛋白酶9(Matrix Metalloproteinase，MMP-9)mRNA的表达情况、临床妊娠率及种植率。结果：治疗后观察组子宫内膜厚度、容积，内膜下血流及PI和RI，VI、FI、VFI与空白组比较，差异无统计学意义(P＞0.05)，观察组、空白组与对照组比较，差异有统计学意义(P＜0.05)；着床期αv β3mRNA、Hox10mRNA、MMP9mRNA表达水平观察组与空白组比较，差异无统计学意义(P＞0.05)；观察组、空白组与对照组比较，差异有统计学意义(P＜0.05)；临床妊娠率和种植率观察组与空白组比较，差异无统计学意义(P＞0.05)；观察组、空白组与对照组比较，差异有统计学意义(P＜0.05)。结论：补肾中药治疗肾阴虚型IVF-ET患者，不仅可改善子宫内膜的厚度、容积、内膜下血流等情况，还可提高子宫内膜容受性，有效地提高患者的临床妊娠率和种植率。
Abstract:
Objective: To explore the effects of kidney-tonifying herbs on endometrial receptivity of in vitro fertilization-embryo transfer (IVF-ET) patients of kidney Yin deficiency pattern. Methods: Eighty patients were allocated to two groups according to random number table method, the blank group and the observation group, 40 cases each group another 40 patients who performed IVF-ET were chosen as the control group because of simple fallopian tube factor, the control group and the blank group were given conventional regimen, the observation group were treated by kidney-tonifying herbs based on conventiaonal regimen. To observe and compare endometrial thickness, volume and flow among three groups, compare the levels of bilateral uterine artery PI, RI, endometrial VI, blood FI and VFI between both groups, simultaneously compare the expressions of integrin (αvβ3) mRNA, Hox10mRNA and observation MMP-9 mRNA, clinical pregnancy rate and implantation rate during implantation period. Results: After treating, endometrial thickness, volume and flow, PI, RI, VI, FI and VFI of the observation group were compared with these of the blank group, and the difference had no statistical meaning (P＞0.05), the difference had statistical meaning when the observation group and the blank group were compared with the control group (P＜0.05); the difference had no statistical meaning when the observationgroup was compared with the blank group in the expressions of αvβ3 mRNA, Hox10 mRNA and MMP-9 mRNA during implantation period(P＞0.05), the difference was statistically significant when the observation group and the blank group were compared with the control group (P＜0.05); The difference had no statistical meaning when the observation group was compared with the blank group in clinical pregnancy group and implantation rate (P＞0.05); the difference was statistically significant when the observation group and the blank group were compared with the control group (P＜0.05). Conclusion: Kidney-tonifying herbs possess good clinical effects in the treatment of IVF-ET patients of kidney Yin deficiency pattern, it could not only improve endometrial thickness, volume, flow and others, but also raise endometrial receptivity, effectively lift clinical pregnancy rate and implantation rate.
