摘要:
目的：从口腔护理结局指标角度来观察白茅根及蒲公英中药冰块在游离舌黏膜行前尿道成形术后患者的口腔护理中的效果。方法：将64例舌黏膜尿道成形术患者随机分为观察组和对照组，每组32例。对照组在舌黏膜尿道成形术后口腔采用传统护理方法；观察组术后采用自制的白茅根蒲公英中药冰块含漱，比较2组患者术后3天并发症发生情况、口腔护理的满意度及术后7天伤口的Ⅰ期愈合率。结果：并发症发生率观察组为9.38%（3/32），对照组为53.13%（17/32），两组比较，差异有统计学意义(P＜0.05)。术后3天患者口腔护理满意度观察组为53.13%（17/32），对照组为21.88%（7/32），两组比较差异有统计学意义(P＜0.05)。术后伤口Ⅰ期愈合率观察组为96.88%（31/32），对照组为71.88%（23/32），两组比较差异有统计学意义(P＜0.05)。术后焦虑评分与流质饮食恢复时间两组比较，差异有统计学意义(P＜0.05)。术后口腔疼痛评分相同时间点两组比较，差异有统计学意义(P＜0.05)。结论：白茅根及蒲公英中药冰块口腔护理方法，提高了游离舌黏膜行前尿道成形术后患者的依从性，降低了口腔并发症，有效地提高了患者的口腔护理结局。
Abstract:
Objective: To observe the effects of oral care with ice of Baimaogen (imperatae rhizoma) and Pugongying (taraxaciherba) on the patients who underwent oral mucosal graft urethroplasty from the angle of outcome indicators of oral care. Methods: A total of 64 patients were randomized into the observation group and the control group, 32 cases each group, the control group accepted traditional nursing method after the urethroplasty, and the observation group gargled with selfmade ice of Baimaogen and Pugongying after the operation, to compare the incidence of the complications within three days after the operation, oral nursing satisfaction degree and the healing rate of stageⅠwound seven days after the operation. Results: The incidence of the complication of the observation group was 9.38% (3/32), lower than 53.13% (17/32) of the control group, the difference had statistical meaning between both groups (P＜0.05). Oral care satisfactory degree three days after the operation in the observation group was 53.13% (17/32), higher than 21.88% (7/32) of the control group, and the difference showed statistical meaning between both groups (P＜0.05). The healing rate of stageⅠpostoperative wound was 96.88% (31/32), higher than 71.88% (23/32) of the control group, and the difference had statistical meaning between both groups (P＜0.05). The difference had statistical meaning in postoperative anxiety score and the recovery time of fluid diet between both groups (P＜0.05). The difference showed statistical meaning in postoperative oral pain scores between both groups at the same time point (P＜0.05). Conclusion: Oral nursing methods with ice of Baimaogen and Pugongying could lift the patients′ compliance after performing anterior urethroplasty of free lingual mucosa, reduce oral complication and effectively improve oral nursing outcome.
