摘要:
目的：从实验室角度分析急性丘脑梗死患者中医证型，了解其与血脂、白细胞介素6(interleukin-6，IL-6)、超敏C反应蛋白(high-sensitivity C-reaction protein，Hs-CRP)、血浆脂蛋白磷脂酶A2 (lipoprotein phospholipase A2，(Lp-PLA2)水平的相关性。方法:对103例患者进行实验室血液指标检测，同时进行中医辨证分型，调查急性丘脑梗死患者中医证候分布规律，分析中医证型与血脂、IL-6、Hs-CRP、Lp-PLA2水平之间的相关性。结果:急性丘脑梗死患者总胆固醇(total cholesterol，TC)、甘油三酯(triglyceride，TG)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol，LDL-C)、Lp-PLA2、IL-6、Hs-CRP水平明显高于正常对照组，而高密度脂蛋白胆固醇(high density lipoprotein cholesterol，HDL-C)水平则低于正常对照组。风证、痰证和瘀证三型TC、TG、LDL-C、Lp-PLA2、IL-6、Hs-CRP水平明显高于其他证型。结论:风、火、痰证、瘀证等实证为急性丘脑梗死发病的关键因素。血脂、IL-6、Hs-CRP、Lp-PLA2水平的异常可能为急性丘脑梗死患者风火相煽、痰瘀并见等病理要素的理化基础之一。
Abstract:
Objective: To analyze TCM patterns in the patients with acute thalamic infarction (ATI) from lab perspective, and investigate its connections with the levels of blood lipid, IL-6, hs-CRP and Lp-PLA2. Methods: Blood indexes of 103 patients were detected, simultaneously, TCM patterns were differentiated and analyzed, to investigate the distribution laws of TCM patterns in ATI patients, to analyze the correlations between TCM patterns and the levels of blood lipid, IL-6, hs-CRP and Lp-PLA2. Results: The levels of TC, TG, LDL-C, Lp-PLA2, IL-6, hs-CRP of the patients were higher than these of normal control group apparently, while the levels of HDL-C were lower than that of normal control group. The levels of TC, TG, LDL-C, Lp-PLA2, IL-6 and Hs-CRP in the patients of wind pattern, phlegm pattern and stagnation pattern were obviously higher than these in other patterns. Conclusion: Wind, fire, phlegm, stagnation and other pattern are the critical factors of ATI. Abnormal blood lipid, IL-6, hs-CRP and Lp-PLA2 might be one of physical and chemical basis for pathological elements wind and fire stirring, phlegm and stagnation and others.
