摘要:
目的：通过瞬时弹性成像技术(transient elastography，TE)评价自拟中药治疗6个月对肝纤维化的改善程度。方法：筛选肝纤维化患者80例，随机分为对照组和观察组，每组40例，对照组予以扶正化瘀胶囊治疗6个月，观察组予以自拟中药治疗6个月。所有患者在治疗前后均进行TE检查并记录肝脏硬度值(liver stiffness measurement，LSM)，检查血清α2巨球蛋白(α2-macroglobulin，α2-MG)、结合珠蛋白(haptoglobin，HAP)、谷氨酰转肽酶(glutamyltranspetidase，GGT)、总胆红素(total bilirubin，TBII)、载脂蛋白Al(apolipoprotein，ApoAl)并计算FibroTest值(FT)。对比两组患者治疗前后的LSM值及FT值，并对其进行相关性分析，对肝纤维化的改善程度进行评价。结果：α2-MG、HAP、ApoAl、TBII、GGT治疗前后两组组内比较，差异均有统计学意义(P＜0.05)；ApoAl、TBIL治疗后两组组间比较，差异有统计学意义(P＜0.05)。两组治疗前后FT值与LSM值治疗前后两组组内比较，差异有统计学意义(P＜0.05)；治疗后两组组间比较差异也有统计学意义(P＜0.05)。治疗前后两组的FT值与LSM值差异有统计学意义(P＜0.05)。结论：TE技术可以应用于肝脏硬度的测量，有利于诊断和评估肝纤维化的程度及分期。自拟中药可降低肝脏LSM值，对于治疗肝纤维化具有较好的疗效，能够阻止及逆转肝脏的纤维化程度。
Abstract:
Objective: To assess the improvement degrees of liver fibrosis treated by selfmade herbs via transient elastography (TE). Methods: Eighty patients were screened, randomized into the control group and the observation group, 40 cases each group, the control group took Fuzheng Huayu capsules for six months, and the observation group were given selfmade herbs for six months. All the patients performed TE examinations before and after treating to record LSM, detect α2-MG, HAP, GGT, TBIL, ApoAI, and calculate FT. To compare LSM and FT before and after treating between both groups, and relevance analysis was performed to assess the improvements of liver fibrosis. Results: The difference had statistical meaning in α2-MG, HAP, ApoAI, TBIL and GGT before and after treating within two groups (P＜0.05); the difference showed statistical meaning in ApoAI and TBIL between both groups after treating (P＜0.05). The difference had statistical meaning in FT and LSM before and after treating within two groups (P＜0.05); the difference was statistically significant between both groups after treating (P＜0.05). FT was significantly correlated with LSM before and after treating in both groups (P＜0.05). Conclusion: TE technology could be applied to measure liver stiffness, and it could help diagnose and assess the degrees and phasing of liver fibrosis. Selfmade herbs could lower LSM of liver, showing better effects for liver fibrosis, it could block and reverse the degree of fibrosis.
