摘要:
目的探讨黄芪注射液对病毒性心肌炎患者外周血核苷酸结合寡聚化结构域样受体蛋白3（nucleotide-binding oligomerization domain-like receptors family pyrin domain containing 3,NLRP3）炎性小体表达的影响。 方法将病毒性心肌炎患者160例根据随机数字表法分为黄芪注射液治疗组（观察组）和常规治疗组（对照组）各80例。观察两组临床疗效及不良反应发生情况，比较治疗前后两组患者外周血单个核细胞（peripheral blood mononuclear cells,PBMCs）中NLRP3、半胱氨酸蛋白酶1（Caspase-1）mRNA表达水平及白细胞介素1β(interleukin-1β，IL-1β)、白细胞介素1(interleukin-1，IL-1)、心肌肌钙蛋白I（cardiac troponin I,cTnI）和肌酸激酶同工酶MB（creatine kinase isoenzymes-MB,CKMB）水平。 结果与对照组比较，观察组治疗后外周血NLRP3、Caspase-1 mRNA及IL-1β和IL-18水平降低（P＜0.05）；治疗后两组患者血清CKMB、cTnI水平均较治疗前降低，观察组降低更明显（P＜0.05）；观察组总有效率高于对照组（P＜0.05）；不良反应两组比较差异无统计学意义（P＞0.05）。 结论黄芪注射液可降低病毒性心肌炎患者外周血NLRP3炎性小体表达水平，从而提高临床疗效。
Abstract:
ObjectiveTo observe the effect of Huangqi injection on the expression of NLRP3 inflamma-some in peripheral blood of patients with viral myocarditis. MethodsAll 160 patients were divided into the treatment group of Huangqi injection (the observation group) and conventional treatment group (the control group) according to randome number table method, 80 cases in each group. To observe clinical effects and the incidences of adverse reaction in two groups, to compare the expressions of NLRP3 in PBMCs, Caspase-1 mRNA, the levels of IL-1β, IL-1, CKMB and cTn1 in peripheral blood of the patients between both groups before and after treating. ResultsCompared with the control group, the levels of NLRP3, Caspase-1 mRNA, IL-1β and IL-18 in peripheral blood lowered in the observation group (P<0.05); the levels of CKMB and cTn1 reduced in both groups after treating than these before treating, and the decrease of the observation group was more notable (P<0.05); the observation group was higher than the control group in total effective rate (P<0.05); the difference had no statistical meaning between both groups in adverse reaction (P>0.05). ConclusionHuangqi injection could lower the expressions of NLRP3 in peripheral blood of the patients with viral myocarditis, therefore to raise clinical effects.
