摘要:
目的观察安胰颗粒联合麻杏石甘汤治疗重症急性胰腺炎（severe acute pancreatitis，SAP）并发急性肺损伤（acute lung injury，ALI）的疗效。 方法将SAP并发ALI患者60例按随机分配方法分为对照组和治疗组各30例，对照组予西医常规治疗，治疗组在常规治疗基础上口服安胰颗粒+麻杏石甘汤。观察两组患者疗效、急性生理与慢性健康评分（acute physiology and chronic health evaluation，APACHE-Ⅱ），检测血氧分压与吸氧浓度的比值（PaO2/FiO2），同时检测患者静脉血中白细胞（white blood cell，WBC）、C反应蛋白（C-reaction protein，CRP）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、白细胞介素6（interleukin-6，IL-6）的含量。 结果1）总有效率治疗组为93.33%，对照组为70.00%，差异有统计学意义（P＜0.05）；2）与治疗前对比，治疗后两组患者PaO2/FiO2、APACHE-Ⅱ评分下降（P＜0.01），治疗后组间比较差异有统计学意义（P＜0.01）；3）与治疗前对比，治疗后两组患者WBC均降至正常（P＜0.01），CRP、TNF-α、IL-6含量明显下降（P＜0.01），组间比较差异有统计学意义（P＜0.01）。 结论在常规治疗的基础上口服安胰颗粒联合麻杏石甘汤能有效抑制SAP并发ALI患者的炎症反应、改善微循环，促进恢复患者肺功能。
Abstract:
ObjectiveTo observe clinical effects of Anyi granules and Maxing Shigan decoction in treating the patients suffering SAP complicated with ALI. MethodsSixty patients were separated into the control group and the treatment group according to random assignment method, 30 cases in each group, the control group was given conventional therapy of Western medicine, and the treatment group took Anyi granules and Maxing Shigan decoction on the foundation of conventional therapy. To observe clinical effects and APACHE-II scores of the two groups, detect PaO2/FiO2, simultaneously, to measure the contents of WBC, CRP, TNF-α and IL-6 in venous blood. Results1) Total effective rate of the treatment group was 93.33%, higher than 70.00% of the control group, and the difference had statistical meaning (P<0.05); 2) Compared with before treating, PaO2/FiO2 and APACHE-II scores lowered in both groups after treating (P<0.01), the difference showed statistical meaning between both groups after treating (P<0.01); 3) Compared with before treating, WBC of both groups decreased to normal after treating (P<0.01), the contents of CRP, TNF-α and IL-6 lowered notably (P<0.01), the difference had statistical meaning (P<0.01). ConclusionAnyi granules and Maxing Shigan decoction could effectively inhibit inflammatory reaction, improve microcirculation and promote the recovery of lung function in the treatment for the patients suffering SAP complicated with ALI.
