摘要:
目的观察下瘀血汤灌肠联合穴位贴敷治疗恶性肿瘤患者阿片药物相关性便秘的临床疗效。 方法将60例阿片类药物相关性便秘患者按照数字表法随机分为观察组及对照组各30例。两组均给予规范化镇痛治疗，观察组予以中药灌肠联合穴位贴敷，对照组采用乳果糖溶液口服。观察两组临床疗效、便秘症状积分、生存质量评分及不良反应发生率。 结果观察组总有效率［90.0%（27/30）］高于对照组［66.7%（20/30）］（P＜0.05）。观察组排便困难、粪便性状、肛门坠胀或不尽感评分均较治疗前降低（P＜0.05），且排便困难、粪便性状评分低于对照组（P＜0.05）；对照组便秘症状各项目评分治疗前后无明显变化（P＞0.05）。两组生存质量评分均较治疗前降低（P＜0.05），且观察组治疗后生存质量评分低于对照组（P＜0.05）。观察组不良反应发生率［6.7%（2/30）］与对照组［3.3%（1/30）］比较，差异无统计学意义（P＞0.05）。 结论下瘀血汤能有效缓解阿片类药物相关性便秘患者的症状，提高患者的生活质量，且安全性较好。
Abstract:
ObjectiveTo observe clinical effects of blood stasis-removing decoction enema jointed with acupoint application in the treatment of opioid-induc constipation. MethodsSixty patients were divided into the observation group and the control group according to random number table method, 30 cases in each group, both groups adopted standardized analgesic therapy, the observation group was given herbal enema and acupoint application, and the control group took lactulose solution orally. To observe clinical effects, the scores of consti-pation symptoms, the scores of quality of life and the incidence of adverse reaction in both groups. ResultsTotal effective rate of the observation group was 90.0% (27/30), higher than 66.7% (20/30) of the control group (P<0.05). The scores of difficult defecation, fecal characteristics, anal distension or incompleteness, defecation frequency and total scores of the observation group lowered compared with these before treating (P<0.05), the scores of difficult defecation, fecal characteristics, and total scores were lower than these of the control group (P<0.05). the scores of PAC-QOL lowered in both groups after treating (P<0.01), and the scores of PAC-QOL of the observation group were lower than these of the control group (P<0.05). The incidences of adverse reaction of the observation group and the control group were 6.7% (2/30) and 3.3% (1/30), and the difference had no statistical meaning (P>0.05). ConclusionBlood stasis-removing decoction could allievate the symptoms of the patients with opioid-induced constipation, raise quality of life with better safety.
