摘要:
目的运用数据挖掘技术分析蓝青强教授治疗慢性胃炎（chronic gastritis，CG）的处方用药规律，为CG临床用药提供参考。 方法选择蓝青强教授门诊治疗CG的病例为研究对象，收集病例相关信息及处方用药，建立方药数据库，录入中医传承辅助平台软件（TCMISS V 2.5）进行数据分析，通过关联规则、互信息法、熵聚类算法分析等数据挖掘技术获得蓝青强教授治疗CG的高频中药及核心配伍，从中探寻用药规律及发掘候选新方。 结果共纳入207首处方，使用频次≥40次的中药共21味包括黄芪、白术、太子参、茯苓、当归、陈皮、法半夏、柴胡、淫羊藿、苍术等；其中以补益药、利水渗湿药、理气药为主，临证加减使用活血化瘀药、清热药、化湿药、祛风湿药、止血药、消食药、安神药、泻下药等；药性以温、凉、平为主，药味以甘、淡、苦为主，归经以脾、胃、肝经为主。常用药对39对包括茯苓-白术、茯苓-黄芪、茯苓-太子参等；强关联规则24条包括茯苓、白术→黄芪，茯苓、白术→丹参等；候选新处方8个包括白芍、柴胡、紫苏梗、太子参、法半夏等。 结论蓝青强教授治疗CG临证用药以健运脾胃为本，消滞和胃为标，重视调气活血，疏肝利胆，选用药物多归脾、胃、肝经，性味多温、凉、甘，功效以补益、渗湿、理气为主，随证加减活血化瘀、清热、祛风、止血、消食、安神等药。
Abstract:
ObjectiveTo provide the reference for clinical medication of chronic gastritis (CG) by analyzing professor Lan Qingqiang's medication rules of treating CG through using data mining technology. MethodsProfessor Lan Qingqiang's medical cases of treating CG were chosen as the research objects, the relevant information and medication of medical cases were collected to establish the databases of prescriptions and materia medica, the information was typed into TCMISS V2.5 to carry out data analysis, high-frequency herbs, core compatibility of professor Lan's treatment for CG were obtained by association rules, mutual information method and entropy clustering algorithm, therefore to explore the medication laws and unearth the candidate prescription. ResultsAll 207 prescriptions are included, 21 herbs with the frequency≥40 times are Huangqi (Radix Astragali), Baizhu (Rhizoma Atractylodis Macrocephalae), Taizishen(Radix Pesudostellariae), Fuling [Wolfiporia cocos(Schw.) Ryv.&Gibn], Danggui (Radix Angelica Sinensis), Chenpi (Pericarpium Citri Reticulatae), processed Banxia (Rhizoma Pinellinae Praeparata), Chaihu (Root of Chnese Thorowax), Yinyanghuo (Epimedii folium), Cangzhu (Rhizoma of Swordlike Atractylode), etc; among which, most are tonics, the herbs of clearing damp and promoting diuresis, the herbs of regulating Qi, modifying and using the herbs of blood-activating and stasis-resolving, heat-clearing, damp-resolving, dispelling wind-damp, stopping bleeding, helping digestion, tranquilizer and purgating drug according to the symptoms; the drugs are warm, cool and neutral, the drugs’ flavors are sweet, light and bitter mainly, the meridian tropism belongs to spleen, stomach and liver primarily. There were 39 commonly-used drug pairs, containing Fuling-Baizhu, Fuling-Huangqi, Fuling-Taizishen, etc; there are 24 strong association rules, including Fuling, Baizhu→Huangqi, Fuling, Baizhu→Danshen (Danshen Root), etc; eight new candidate prescriptions involve Baishao (Chinese Peony), Chaihu, Zisugeng (Caulis Perillae), Taizishen, processed Banxia, etc. ConclusionProfessor Lan Qingqiang's clinical medication for the treatment of CG is based on invigorating spleen and stomach, eliminating stagnation and harmonizing stomach as the method, he pays attention to regulating Qi and activating blood, soothing liver and gallbladder, selects the drugs entering the meridians of spleen, stomach and liver, of warm, cool and sweet, its efficacy is tonifying, excreting dampness and regulating Qi mainly, modifying the herbs of blood-activating and stasis-resolving, heat-clearing, dispelling wind, stopping bleeding and helping digestion, tranquilizer in light of the symptoms.
