摘要:
分析甘肃省新型冠状病毒肺炎（Corona Virus Disease 2019，COVID-19）（简称新冠肺炎）临床病例的中医发病特点，辨证论治经验。新冠肺炎属于中医“温疫”范畴，甘肃省新冠肺炎具有“热郁证突出，兼夹湿邪”的特点。风湿之邪直接犯肺，郁于肺而化热，肺失宣降；风湿并重，阻滞经络，气血运行不利，致热毒闭于肺；热毒由外及里，疫入心包，出现内闭外脱之证；阳邪耗气伤阴，累及脾肺，出现肺脾气虚之象。基于此，甘肃省新冠肺炎的辨证可分为温邪犯肺、温热壅肺、温毒闭肺、内闭外脱、脾肺气虚五个阶段，治疗当以宣肺散邪、清热解毒、分解湿热、运脾益肺、扶正祛邪为原则。
Abstract:
Based on clinical cases in Gansu province, the experience in syndrome differentiation and treatment, the characteristics of pathogenesis and the morbidity of COVID-19 were analyzed in Gansu province. COVID-19 belongs to the category of "warm pestilence" in TCM, and COVID-19 shows the pattern of "heat depression prominent, and damp evil". Wind-damp evils directly invade the lung, stagnating in the lung and transforming into the heat, therefore, lung fails to diffuse and descend; wind and damp obstruct the meridian and collateral so that Qi and blood cannot move smoothly, which could induce heat-toxin closing in the lung; heat-toxin enters into the pericardium by the outside and the inside, and the patients manifesting the pattern of internal blocking causing collapse; Yang evil consuming Qi and hurting Yin, involving spleen and lung, and the patients appearing the pattern of Qi deficiency of lung and spleen. Based on this, COVID-19 could be differentiated into five stages: warm evil invading the lung, warm-heat evil stagnating in the lung, warm-toxin closing the lung, internal blocking causing collapse and Qi deficiency of spleen and lung, and the treatment should adopt diffusing lung and dispersing the evil, clearing heat and detoxifying, resolving dampness and heat, activating spleen and benefiting lung, enhancing healthy Qi and eliminating the pathogenic factors as the principles.
