摘要:
目的探讨细胞因子信号转导抑制因子3（suppressor of cytokine signaling 3，SOCS3）介导的蛋白酪氨酸激酶1（janus protein kinase 1，JAK1）/信号转导与转录激活子5（signal transducer and activator of transcription 5，STAT5）炎症通路表达对慢性湿疹血虚风燥证患者的影响及壮医脐环穴针刺法对其的干预作用。 方法选择62例慢性湿疹血虚风燥证患者，随机分为对照组和观察组各31例，实际完成对照组30例，观察组28例，并选取26名健康体检者作为正常组。对照组给予复方甘草酸苷片口服和糠酸莫米松乳膏外涂治疗，观察组采用壮医脐环穴针刺法治疗，疗程为4周。观察两组临床疗效，湿疹面积及严重度指数评分（eczema area and severity index，EASI），视觉模拟评分（visual analogue scale，VAS），外周血单个核细胞（peripheral blood mononuclear cell，PBMC）中SOCS3、JAK1、磷酸化蛋白酪氨酸激酶1、STAT5、磷酸化信号转导与转录激活子5（pSTAT5）蛋白表达，血清白细胞介素4（interleukin-4，IL-4）、白细胞介素6（interleukin-6，IL-6）、白细胞介素10（interleukin-10，IL-10）水平。 结果观察组临床总有效率［85.71%（24/28）］优于对照组［60.00%（18/30）］（P＜0.05）。治疗后，两组EASI、VAS评分较治疗前均降低（P＜0.05），且观察组降低降程大于对照组（P＜0.05）。两组治疗前PBMC中SOCS3、JAK1、pJAK1、STAT5、pSTAT5蛋白表达量均高于正常组（P＜0.05）；治疗后两组SOCS3蛋白表达量均较治疗前升高，JAK1、pJAK1、STAT5、pSTAT5蛋白表达量降低（P＜0.05），观察组改善程度优于对照组（P＜0.05）。两组治疗前血清IL-4、IL-6、IL-10水平均高于正常组（P＜0.05），治疗后两组上述指标均较治疗前降低（P＜0.05），且观察组低于对照组（P＜0.05）。 结论壮医脐环穴针刺法能改善慢性湿疹血虚风燥证患者的临床症状，提高临床疗效，其机理与调控细胞内SOCS3-JAK1/SATA5信号通路表达，抑制患者的炎症反应有关。
Abstract:
ObjectiveTo discuss the influence of SOCS3-JAK1/SATA5 signaling pathway expressions on the patients with chronic eczema of blood deficiency and wind dryness pattern, and the intervention effects of acupuncture method of umbilical surrounded acupoints on the patients. MethodsAll 62 patients were randomized into the control group and the observation group, 31 cases in each group, actually 30 cases of the control group and 28 cases of the observation group finished the therapy, 26 healthy controls were selected as the normal group. The control group took compound glycyrrhizin tablets orally and received mometasone furoate cream for external use, the observation group adopted acupuncture method of umbilical surrounded acupoints in Zhuang medicine, the course was four weeks. To observe clinical effects, EASI, VAS, the expressions of SOCS3, JAK1, phosphorylate janus protein kinase 1(pJAK1), STAT5, pSTAT5 protein, the levels of IL-4, IL-6 and IL-10 in PBMC between both groups. ResultsTotal effective rate of the observation group was [85.71%(24/28)], higher than [60.00%(18/30)] of the control group (P<0.05). After the treatment, EASI and VAS scores decreased in both groups than these before the treatment (P<0.05), and the decrease of the observation group was greater than that of the control group (P<0.05). Before the treatment, the expressions of SOCS3, JAK1, pJAK1, STAT5 and pSTAT5 protein in both groups were higher than these of the normal group (P<0.05); after the treatment, SOCS3 protein expressions of both groups were increased than these before treatment, the expressions of JAK1, pJAK1, STAT5 and pSTAT5 were decreased (P<0.05), the improvements of the observation group were superior to these of the control group (P<0.05). The levels of IL-4, IL-6 and IL-10 of both groups were higher than these of the normal group before treatment (P<0.05), after the treatment, the above indexes of both groups were reduced than these before treatment (P<0.05), and the observation group was lower than the control group (P<0.05). ConclusionZhuang medicine acupuncture method of umbilical surrounded acupoints could improve clinical symptoms in patients, raise clinical effects, and its mechanism is related to regulating the expressions of SOCS3-JAK1/SATA5 signaling pathway in the cells, and inhibiting the patients' inflammatory reaction.
