摘要:
目的探讨补肾健脾化瘀中药对难治/复发再生障碍性贫血(aplastic anemia,AA)患者骨髓CD3+T淋巴细胞内磷酸化哺乳动物雷帕霉素靶蛋白(phosphorylated mammalian target of rapamycin,p-mTOR)、磷酸化核糖体蛋白S6(phosphorylated ribosomal protein S6,p-S6)及干扰素γ(interferon-γ,IFN-γ)的影响,解析其治疗难治/复发AA的机制。 方法采集初发AA 13例、难治/复发AA 15例及对照组14例研究者的骨髓液,分别加入补肾健脾化瘀中药、雷帕霉素进行孵育,运用流式细胞术检测药物处理前后各组CD3+T淋巴细胞胞质内p-mTOR、p-S6与IFN-γ表达水平。 结果药物处理前,复发/难治AA患者CD3+T淋巴细胞p-mTOR、p-S6、IFN-γ表达均高于初发AA与对照组(P<0.01);初发AA患者与对照组比较p-mTOR、p-S6表达差异无统计学意义(P>0.05);初发AA患者IFN-γ表达高于对照组(P<0.01)。药物处理后,复发/难治AA组中,补肾健脾化瘀中药及雷帕霉素均能降低CD3+T淋巴细胞p-mTOR、p-S6与IFN-γ表达(P<0.01),雷帕霉素与补肾健脾化瘀中药组比较各指标水平差异无统计学意义(P>0.05);初发AA与对照组中,雷帕霉素、补肾健脾化瘀中药不能明显降低CD3+T淋巴细胞p-mTOR、p-S6表达(P>0.05);初发AA组中,雷帕霉素、补肾健脾化瘀中药能降低CD3+T淋巴细胞IFN-γ表达(P<0.01);对照组中,雷帕霉素、补肾健脾化瘀中药不能降低CD3+T淋巴细胞IFN-γ表达(P>0.05)。 结论补肾健脾化瘀中药通过抑制难治/复发AA患者CD3+T淋巴细胞内p-mTOR、p-S6、IFN-γ表达,进而抑制T淋巴细胞的异常激活,改善骨髓造血功能。
Abstract:
ObjectiveTo analyze the mechanism of medicine of tonifying-kidney invigorating-spleen eliminating-stasis in the treatment of refractory/recurrent aplastic anemia (AA) by discussing the effects of the medicine on the expressions of p-mTOR, p-S6 and IFN-γ in CD3+T lymphocytes in marrow of refractory/recurrent AA patients. MethodsAfter collecting the marrow fluid from 13 cases of incipient AA, 15 cases of refractory/recurrent AA and 14 cases in the control group, medicine of tonifying-kidney invigorating-spleen eliminating-stasis and rapamycin were respectively added to perform the incubation. Flow cytometry was used to detect the expressions of p-mTOR, p-S6 and IFN-γ in CD3+T lymphocytes in different groups before and after processing with medicine. ResultsBefore medication, the expressions of p-mTOR, p-S6 and IFN-γ in CD3+T lymphocytes in refractory/recurrent AA were higher than these of incipient AA and the control group (P<0.01); the difference had no statistical meaning in the expressions of p-mTOR and p-S6 between the patients with incipient AA and the control group (P>0.05); the expressions of IFN-γ in patients with incipient AA were higher than these of the control group (P<0.01). After medicating, in group of refractory/recurrent AA, medicine of tonifying-kidney invigorating-spleen eliminating-stasis and rapamycin could reduce the expressions of p-mTOR, p-S6 and IFN-γ in CD3+T lymphocytes (P<0.01); while in the group of incipient AA and the control group, rapamycin and herbs could not obviously lower the expressions of p-mTOR and p-S6 in CD3+T lymphocytes (P>0.05); in the group of incipient AA, both of the medicine could reduce the expression of IFN-γ in CD3+T lymphocytes (P<0.01); in the control group, both of the medicine could not obviously lower the expression of IFN-γ in CD3+T lymphocytes (P>0.05). ConclusionMedicine of tonifying-kidney invigorating-spleen eliminating-stasis could inhibit the abnormal activation of T lymphocyte and improve bone marrow hematopoietic function through restraining the expressions of p-mTOR, p-S6 and IFN-γ in CD3+T lymphocytes in patients suffering refractory/recurrent AA.
