摘要:
目的通过古今医案云平台探讨全国名中医廖志峰教授“健胃和中”学术思想及其临床应用特点。 方法将2007—2021年廖志峰教授诊疗的消化系统疾病部分病例469份处方录入古今医案云平台，通过优势病种、中药性味归经分析廖志峰教授诊治疾病的思路及用药特点。 结果病种方面，主要以胃痛和胃痞2种脾胃病科优势病种为主，其他有胃反、泄泻、胁痛、便秘等。药物使用方面，廖志峰教授处方中使用中药约295种，常用的10种中药使用频次达2378次，占总频次的37.6%，这些药物中，除了调和诸药的甘草，使用频次最高的是降气和胃的半夏，其他如厚朴、陈皮、白术、莱菔子、枳壳等为健胃通降剂；同时这些药物以温平性为主，多归脾、胃经，其次归肺、心、肝经。 结论廖志峰教授诊疗疾病以消化系统疾病占比较重，且重用健胃助运及平衡中和中药，体现了其“健胃和中”的学术思想及其调阴阳虚实的诊疗思路。
Abstract:
ObjectiveTo investigate professor Liao Zhifeng's academic thoughts of “strengthening stomach and harmonizing the middle energizer” and the characteristics of its clinical application by ancient and modern medical record cloud platform. MethodsProfessor Liao Zhifeng's thinking of diagnosing and treating the disease and his medication features were analyzed from dominant disease entities, the property, flavor, meridian tropism of materia medica after choosing nearly 469 prescriptions of several periods from the professor's medical cases of digestive system diseases between 2007 and 2021, and typing into ancient and modern medical record cloud platform. ResultsThe aspects of disease entities, stomachache and gastric distension are two dominant diseases in the Department of spleen and stomach disease, the others contain regurtation, diarrhea, hypochondriac pain, constipation and others. The aspects of drug use, professor Liao has used around 295 herbs in the prescriptions, and ten kinds of commonly-used drugs are used more than 2378 times, holding 37.6% of overall frequencies, among these herbs, except for Gancao that reconciled different drugs, Banxia (pinelliae rhizoma) is the most frequently used herb, which could descend Qi and harmonize stomach, other drugs such as Houpo (magnoliae officinalis cortex),Chenpi(Citri Reticulatae pericarpium), Baizhu(Atractylodis Macrocephalae Rhizoma), Zhiqiao (Aurantii Fructus) and others belong to the agents of strengthening stomach and activating Qi; meanwhile, these herbs are of warm and mild nature mainly, mostly entering to spleen and stomach meridians, lung, heart and liver meridians the next. ConclusionThe disease diagnosed and treated by professor Liao Zhifeng are mainly of digestive system, he emphasizes the application of the herbs of strengthening stomach and pomoting transportation and balancing neutralization, which reflects his academic thought of "strengthening stomach and harmonizing the middle energizer" and diagnostic and therapeutic thinking of regulating Yin and Yang, deficiency and excess.
