摘要:
目的探讨针刺联合腰背肌核心训练对轻度腰椎间盘突出症（lumbar disc herniation，LDH）患者腰肌功能的影响。 方法选取2019年4月至2020年1月收治的98例轻度LDH患者病历，采用回顾性病历分析方法，并根据康复治疗方案分为对照组和联合组各49例。对照组接受腰背肌核心训练康复治疗，联合组接受针刺联合腰背肌核心训练康复治疗，疗程共4周。比较两组患者临床疗效、满意度、腰椎功能、腰部活动范围及腰疼改善情况。 结果总有效率联合组为97.96%，高于对照组的81.63%（P＜0.05）；治疗后联合组日本骨科学会下腰功能评分、Oswestry功能障碍指数问卷表评分、疼痛视觉模拟评分改善优于对照组（P＜0.05），腰部前屈、后伸、左屈、右屈角度大于对照组（P＜0.05）；患者满意度联合组为100.00%，优于对照组的79.59%（P＜0.05）。 结论针刺联合腰背肌核心训练在缓解轻度LDH患者疼痛、改善腰肌功能以及构建和谐医患关系方面优于单纯腰背肌核心训练。
Abstract:
ObjectiveTo explore the effects of needling method in combination with core training of lumbodorsal muscles on lumbar muscle function in patients with mild LDH. MethodsMedical records of 98 cases of mild LDH, admitted to hospital from April, 2019 to January, 2020 were chosen by adopting retrospective medical record analysis method, and divided into the control group and the combination group according to rehabilitation treatment. The control group accepted rehabilitation therapy of core training of lumbodorsal muscles, and the combination group needling method and core training of lumbodorsal muscles, the course of the treatment was four weeks. To compare clinical effects, satisfaction degree, lumbar function, lumbar range of motion (ROM) and the improvements of lumbago, between both groups. ResultsTotal effective rate of the combination group was 97.96%, higher than 81.63% of the control group (P<0.05); after the treatment, the improvements of JOA, ODI scores and VAS of the combination group were superior to these of the control group (P<0.05), greater than the control group in the angles of forward flexion, backward extension, left flexion and right flexion (P<0.05); the patients' satisfaction degree of the combination group was 100.00%, higher than 79.59% of the control group (P<0.05). ConclusionNeedling method joined with core training of lumbodorsal muscles are better than single core training of lumbodorsal muscles in relieving pain, lumbar muscle function and building a harmonious doctor-patient relationship.
