摘要:
目的探讨火针联合祛风除湿止痒汤治疗慢性手部湿疹的作用机制。 方法将102例慢性手部湿疹患者按照随机数字表法分为对照组和观察组各51例。对照组予0.1%他克莫司软膏治疗，观察组予火针联合祛风除湿止痒汤治疗。治疗1周后比较两组患者血清白细胞介素2（interleukin-2，IL-2）、IL-4和干扰素γ（Interferon-γ，IFN-γ）水平；观察两组患者手部湿疹严重程度评分（the hand eczema severity index，HECSI）、Th1、Th2、Th1/Th2比值及临床疗效。 结果治疗后两组患者血清IL-2、IFN-γ水平及Th1、Th1/Th2水平较治疗前升高，IL-4、Th2水平及HECSI评分较治疗前降低（P＜0.05），观察组改善程度优于对照组（P＜0.05）；总有效率观察组［98.04%（50/51）］高于对照组［84.31%（43/51）］（P＜0.05）。 结论火针联合祛风除湿止痒汤治疗慢性手部湿疹临床疗效优于他克莫司软膏，其作用机制可能是改善Th1/Th2免疫平衡。
Abstract:
ObjectiveTo discuss the effects of fire needle combined with wind-dispelling dampness-removing itching-relieving decoction (WDID) on chronic hand eczema (CHE). MethodsAll 102 CHE patients were separated into the control group and the observation group in light of random number table method with 51 cases in each group. The control group was treated with 0.1% tacrolimus ointment, and the observation group accepted fire needle and WDID. To compare the levels of IL-2, IL-4 and IFN-γ in the serum between the two groups after one week of the treatment; to observe the scores of HECS1, Th1, Th2, Th1/Th2 ratio and clinical effects between the two groups. ResultsThe levels of serum IL-2, IFN-γ, Th1 and Th1/Th2 ratio after the treatment were higher than these before the treatment, the levels of IL-4 and Th2, and HECSI scores were lower than these before the treatment (P<0.05), the improvements of the observation group were better than these of the control group (P<0.05), total effective rate of the observation group was [98.04%(50/51)], higher than [84.31%(43/51)] of the control group (P<0.05). ConclusionFire needle combined with WDID in the treatment of CHE is superior to only the ointment, and its mechanism might be related to the improvement of Th1/Th2 immune balance.
