摘要:
目的采用数据挖掘技术，系统分析古代针灸治疗膝骨关节炎的选穴组方规律，为临床研究提供理论依据。 方法以“膝痹”“膝痛”等为检索词，检索《中华医典》第5版“针灸推拿灸部”，提取针灸治疗膝骨关节炎的相关处方条文。使用Excel 2021对条文进行年代、著作名称、腧穴频次、腧穴归经、特定穴、分布、选穴、配穴等方面的整理分析，用SPSS Modeler 18.3进行腧穴、配穴关联规则分析。 结果涉及针灸古籍34本，针灸处方数1004条，单穴处方839条，多穴处方165条，腧穴108个，总频次1356次。宋明时期研究众多，《普济方·针灸》《针灸资生经》等著作为代表性著作；常选用足少阳胆经、足阳明胃经、足太阳膀胱经经穴；重视五输穴之合穴应用；腧穴多分布于小腿、足踝、膝部等；阳陵泉、足三里、委中、犊鼻、环跳应用频次较高；内膝眼-犊鼻支持度最强；三部配穴法-表里经配穴法支持度最高。 结论古代膝骨关节炎的针灸处方以局部取穴为主，重视表里经配穴及本经配穴。
Abstract:
ObjectiveTo provide theoretical reference for clinical study by systematically analyzing acupoint selection rules for ancient acupuncture in the treatment of knee osteoarthritis (KOA) using data mining technology. MethodsUsing "knee arthralgia" "knee pain" and others as the terms, we retrieved the section of "acupuncture, massage and moxibustion" from the 5th edition of Chinese Medical Canon, so as to extract the relevant prescriptions and articles for acupuncture treatment of KOA. Excel 2021 was used to collect and analyze the dynasties, the title of the work, the frequency of acupoints, meridian tropism, specific points, the distribution, acupoint selection and acupoint matching of the articles, SPSS Modeler 18.3 was adopted to analyze the association rules of the acupoint and matching acupoints. ResultsThe study involved 34 ancient books on acupuncture, 1004 prescriptions of acupuncture containing 839 single-acupoint ones, and 165 multiple acupoints prescriptions, 108 acupoints and total frequency reached 1356 times. There were many studies in the Song and Ming dynasties, among which Puji Fang·acupuncture and experience on acupuncture and moxibustion therapy were representative works; the acupoints were frequently chosen from gallbladder meridian of Foot-Shaoyang, stomach meridian of Foot-Yangming and bladder meridian of Foot-Taiyang; importance should be attached to the combined application of Five Shu points; and the acupoints were mostly distributed in the calf, ankle and knee; Yanglingquan (GB34), Zusanli(ST36), Weizhong (BL40), Dubi (ST35) and Huantiao (GB30) were used more frequently; Neixiyan (EX-LE4)-Dubi were with the strongest support; acupoint combination method from three-section-exterior-interior point combination presented the highest support. ConclusionAcupuncture prescription for KOA in ancient times mainly focuses on the selection of adjacent points, paying attention to exterior-interior point combination and point combination of the same meridian.
