摘要:
目的探讨益心通脉汤结合运动想象疗法对急性脑梗死气虚血瘀证患者血清可溶性细胞间黏附分子-1（soluble intercellular adhesion molecule-1，sICAM-1）、中枢神经特异蛋白（S100 calcium-binding protein B，S100b）及肢体功能恢复的影响。 方法选取110例急性脑梗死气虚血瘀证患者，采用随机数表法分为对照组和观察组，每组55例。对照组给予常规药物联合运动想象疗法治疗，观察组在此基础上加用益心通脉汤治疗。连续治疗4周后，比较治疗前后两组患者的中医证候积分、血清sICAM-1、S100b水平、肢体运动功能［上肢Fugl-Meyer运动量表（upper-limb fugl-meyer assessment，U-FMA）、下肢Fugl-Meyer运动量表（lower-limb fugl-meyer assessment，L-FMA）］、平衡能力［Berg平衡量表（berg balance scale，BBS）］及日常生活能力［改良Barthel指数（modified barthel index，MBI）］，并评估两组治疗有效率及安全性。 结果治疗后，两组患者各项中医证候积分及血清sICAM-1、S100b水平均低于治疗前，且观察组更低（P＜0.05）。治疗后，两组U-FMA、L-FMA、BBS、MBI评分均高于治疗前，且观察组更高（P＜0.05）。观察组总有效率［92.73%（51/55）］高于对照组［76.36%（42/55）］（P＜0.05）。两组患者均未出现严重药物不良事件。 结论益心通脉汤结合运动想象疗法有助于改善急性脑梗死气虚血瘀证患者的临床症状，降低血清sICAM-1、S100b水平，可促进肢体运动及平衡能力的恢复，临床疗效显著，安全性良好。
Abstract:
ObjectiveTo investigate the effects of Yixin Tongmai (Heart-benefiting vessel-unobstructing) Decoction combined with motor imagery therapy on serum levels of soluble intercellular adhesion molecule-1 (sICAM-1), central nervous system-specific protein (S100 calcium-binding protein B, S100b), and limb function recovery in patients with acute cerebral infarction of Qi deficiency and blood stasis pattern. MethodsA total of 110 patients with acute cerebral infarction of Qi deficiency and blood stasis pattern were enrolled and randomly divided into a control group and an observation group using a random number table method, with 55 cases in each group. The control group received conventional medication combined with motor imagery therapy, while the observation group was treated with Yixin Tongmai Decoction in addition to the above regimen. After four weeks of continuous treatment, the following outcomes were compared between the two groups before and after treatment: traditional Chinese medicine (TCM) syndrome scores, serum levels of sICAM-1 and S100b, limb motor function assessed by the Upper-Limb Fugl-Meyer Assessment (U-FMA) and Lower-Limb Fugl-Meyer Assessment (L-FMA), balance ability evaluated by the Berg Balance Scale (BBS), and activities of daily living measured by the modified Barthel Index (MBI). Additionally, the treatment response rate and safety profiles were evaluated in both groups. ResultsAfter treatment, both groups showed significant reductions in all TCM syndrome scores and serum levels of sICAM-1 and S100b compared with baseline, with the observation group demonstrating significantly lower values than the control group (P0.05). Meanwhile, the U-FMA, L-FMA, BBS, and MBI scores in both groups were significantly increased after treatment, with the observation group showing significantly higher scores (P0.05). The overall response rate in the observation group was 92.73% (51/55), which was significantly higher than that in the control group [76.36% (42/55)] (P0.05). No severe adverse drug events occurred in either group. Conclusion Yixin Tongmai Decoction combined with motor imagery therapy significantly alleviates clinical symptoms, reduces serum sICAM-1 and S100b levels, and promotes limb motor and balance recovery in acute cerebral infarction patients with Qi deficiency and blood stasis pattern, with notable efficacy and acceptable safety.
