探讨疏肝补肾法针刺联合枸橼酸氯米芬片治疗多囊卵巢综合征（polycystic ovary syndrome，PCOS）伴排卵功能障碍（ovulation dysfunction，OD）的临床疗效，及对血清抗米勒管激素（anti-Müllerian hormone，AMH）、抑制素B（inhibin B，INH-B）水平的影响。 方法选取PCOS合并OD患者60例，采用随机数字表法分为对照组与观察组，各30例。两组均予以健康宣教、生活方式干预、调整月经周期、纠正高雄激素状态等常规基础干预；对照组口服枸橼酸氯米芬片治疗，观察组在对照组治疗基础上加用疏肝补肾法针刺干预，两组均连续治疗3个月经周期。比较两组临床总有效率、最大卵泡直径、卵巢体积、排卵率、妊娠率，以及血清卵泡刺激素（follicle-stimulating hormone，FSH）、黄体生成素（luteinizing hormone，LH）、雌二醇（estradiol，E2）、AMH、INH-B 水平。 结果治疗后，总有效率观察组［93.33%（28/30）］高于对照组［66.67%（20/30）］（P＜0.05）；两组患者卵泡最大直径均增加（P＜0.05），卵巢体积均减小（P＜0.05），且观察组上述指标改善程度大于对照组（P＜0.05）；两组患者血清FSH、LH水平均降低（P＜0.05），E2水平均增加（P＜0.05），且观察组上述指标改善程度大于对照组（P＜0.05）；两组患者血清AMH与INH-B水平均降低（P＜0.05），且观察组降低更明显（P＜0.05）；6个月随访结果显示，排卵率与妊娠率对照组分别为63.33％（19/30）、50.00％（15/30），观察组分别为90.00％（27/30）、80.00％（24/30），观察组排卵率与妊娠率均高于对照组（P＜0.05）。 结论疏肝补肾法针刺联合枸橼酸氯米芬片治疗PCOS伴OD的临床疗效果显著，能增加排卵率与妊娠率，改善性激素水平，并抑制血清AMH与INH-B水平。
Abstract:
ObjectiveTo investigate the clinical efficacy of acupuncture with liver-soothing and kidney-tonifying method combined with clomiphene citrate tablets in the treatment of polycystic ovary syndrome (PCOS) with ovulation dysfunction (OD), as well as its effects on serum levels of anti-Müllerian hormone (AMH) and inhibin B (INH-B). MethodsA total of 60 patients with PCOS complicated by OD were selected and randomly divided into a control group and an observation group using a random number table method, with 30 cases in each group. Both groups received routine basic interventions including health education, lifestyle intervention, menstrual cycle regulation, and correction of hyperandrogenism. The control group was treated with oral clomiphene citrate tablets, while the observation group received additional acupuncture with liver-soothing and kidney-tonifying method based on the treatment of the control group. Both groups were treated continuously for three menstrual cycles. Comparisons were made between the two groups in terms of total clinical effective rate, maximum follicle diameter, ovarian volume, ovulation rate, pregnancy rate, and serum levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), AMH, and INH-B. ResultsAfter treatment, the total effective rate in the observation group [93.33% (28/30)] was significantly higher than that in the control group [66.67% (20/30)] (P0.05). The maximum follicle diameter increased in both groups (P0.05), while ovarian volume decreased in both groups (P0.05), with greater improvements in the observation group than in the control group (P0.05). Serum levels of FSH and LH decreased in both groups (P0.05), while E2 levels increased (P0.05), and the improvements were more pronounced in the observation group than in the control group (P0.05). Serum levels of AMH and INH-B decreased in both groups (P0.05), with a more significant reduction in the observation group (P0.05). Six-month follow-up results showed that the ovulation rate and pregnancy rate in the control group were 63.33% (19/30) and 50.00% (15/30), respectively, while those in the observation group were 90.00% (27/30) and 80.00% (24/30), respectively, indicating that both rates were significantly higher in the observation group than in the control group (P0.05). ConclusionThe acupuncture with liver-soothing and kidney-tonifying method combined with clomiphene citrate tablets demonstrates significant clinical efficacy in the treatment of PCOS with OD. It can increase ovulation rate and pregnancy rate, improve sex hormone levels, and suppress serum levels of AMH and INH-B.
